症状性迟发型性腺功能减退病人下尿路症状调查

目的通过分析症状性迟发型性腺功能减退症(SLOH)病人的下尿路症状(LUTS)患病情况,探讨SLOH与LUTS之间的相关性。方法采用多阶段随机整群不等比例的抽样方法,抽取3000例有固定性伴侣的40～80岁社区中老年男性,采用老年男性症状(AMS)量表及国际前列腺症状评分(IPSS)评估SLOH及LUTS的严重程度,采用Logistic回归分析SLOH与LUTS的相关性。结果共获得符合标准的调查对象2623例,其中SLOH的患病率为39. 04%(1024/2623),LUTS患病率为63. 82%(1674/2623)。伴随着年龄的增长,SLOH及LUTS的患病率均显著增加。无LUTS症状的人群中SLOH患病率为16. 86%,轻度LUTS症状人群的SLOH患病率为42. 89%,而中重度LUTS人群SLOH的患病率为82. 37%。随着LUTS症状程度的增高,SLOH的严重程度明显升高。Logistic回归分析显示:增龄、LUTS严重程度是SLOH的独立影响因素(P<0. 01)。结论中老年男性LUTS病人中存在很高的SLOH发生率。年龄越大,LUTS症状愈严重者SLOH的发生风险更高,临床上在治疗LUTS的同时应该考虑SLOH问题,有效改善其生活质量。

Symptomatic late-onset hypogonadism in men with lower urinary tract symptoms

Objective To investigate the incidence of symptomatic late-onset hypogonadism(SLOH)in men with lower urinary tract symptoms(LUTS),and to analyze the correlation between SLOH and LUTS.Methods A cross-sectional randomized study of 3000 males were investigated using structured questionnaires including International Prostate Symptom Score(IPSS)and aging male symptoms(AMS)in community.Univariate Logistic regression analysis was used to explore the relationship between SLOH and LUTS.Results The final study population consisted of 2623 men.The incidence of SLOH or LUTS was 39.04%and 63.82%,respectively.The incidence of SLOH was 16.86%,42.89%and 82.37%in the normal group,mild SLOH group and moderate-severe SLOH group,respectively(P<0.01).Logistic regression analysis revealed that aging and LUTS were significantly related to SLOH(P<0.01).Conclusions The incidence of SLOH is high in men with LUTS.The patients with an old age or severe LUTS might have a higher risk of SLOH.
