血小板参数与老年冠心病病人冠状动脉病变程度的相关性研究

目的探讨老年冠心病病人血小板参数与冠状动脉病变程度的相关性,为早期评估老年冠心病病人病情提供新的临床思路。方法选取2016年1月至2017年12月在我院心脏中心经冠脉造影确诊为冠心病的老年病人320例,按Gensini评分分为高分组(Gensini评分≥40)和低分组(Gensini评分<40),检测并比较2组间血小板参数的差异。采用Spearman相关分析、多元Logistic回归分析探讨血小板参数与冠状动脉病变程度的相关性。结果与Gensini评分低分组病人相比,高分组病人年龄、合并糖尿病比例、入院时血压、甘油三酯及低密度脂蛋白胆固醇(LDL-C)水平、血小板分布宽度(PDW)均较高(P<0. 05)。Spearman相关分析提示,PDW与Gensini评分呈正相关(r=0. 65,P<0. 05)。Logistic回归分析提示,糖尿病史、LDL-C、PDW是冠状动脉狭窄程度的危险因素(P<0. 05)。结论在老年冠心病病人中,PDW与冠状动脉病变程度成正相关,且为冠状动脉病变严重程度的独立危险因素。

Association of plasma platelet parameters with coronary lesion severity in the elderly patients with coronary heart disease

Objective To investigate the association of plasma platelet parameters with coronary lesion severity in the elderly patients with coronary heart disease(CHD).Methods A total of 320 elderly CHD patients diagnosed by coronary angiography in our hospital from January 2016 to December 2017 were selected and divided into high score group(Gensini score≥40)and low score group(Gensini score<40).The levels of plasma platelet parameters in two groups were detected and compared.Spearman correlation analysis and multi-logistic regression analysis were used to analyze the relationship between the levels of platelet parameters and the degree of coronary artery disease.Results Compared to low score group,the age,ratio of diabetes mellitus,plasma levels of triglyceride(TG)and low density lipoprotein-cholesterol(LDL-C)and platelet distribution width(PDW)in high score group were significantly higher(P<0.05).Spearman correlation analysis showed that PDW was positively correlated to Gensini scores(r=0.65),and multi-logistic regression analysis showed that DM,LDL-C,PDW were independent risk factors for coronary lesion severity.Conclusions PDW is positively correlated to the Gensini scores and it is the independent risk factors for the elderly patients with CHD.
