养阴清热汤联合甲巯咪唑治疗初发弥漫性毒性甲状腺肿临床研究

目的:观察养阴清热汤治疗初发弥漫性毒性甲状腺肿(Graves病)的临床疗效及对甲状腺功能的影响。方法:将诊断的77例Graves病患者随机分为治疗组(n=38)和对照组(n=39),对照组单纯使用甲巯咪唑片治疗,治疗组在对照组的基础上加用养阴清热汤,总疗程为12周。观察两组的临床疗效、不良反应,检测两组治疗前后甲状腺功能、TRAb水平变化并比较。结果:治疗后治疗组和对照组的总有效率分别为94.74%、87.20%,差异有统计学意义(P<0.05)。治疗后两组血清FT3、FT4明显下降,TSH水平明显上升,且治疗组改善程度优于对照组(P<0.05)。治疗组治疗后TRAb水平较治疗前显著降低(P<0.05)。结论:养阴清热汤联合甲巯咪唑治疗初发Graves病可以较快地缓解患者临床症状,改善患者甲状腺功能,降低TRAb水平,减少免疫缓解时间,减少患者痛苦,且不良反应低。

Clinical observation on treatment of initial graves disease with Yangyin Qingre decoction and methimazole

Objective:To observe the clinical efficacy of Yangyin Qingre decoction in the treatment of initial Graves disease and its effect on thyroid function.Methods:77 patients with Graves disease who were newly diagnosed were randomly divided into treatment group and control group.The control group was treated with methimazole tablets alone.The treatment group was supplemented with Yangyin Qingre decoction on the basis of the control group for a total course of 12 weeks.The clinical efficacy and adverse reactions of the two groups were observed.The changes of thyroid function and TRAb levels before and after treatment were detected and compared.Results:After treatment,the total effective rates of the treatment group and the control group were 94.74% and 87.20%,respectively.After treatment,serum FT 3 and FT 4 decreased significantly,TSH levels increased significantly,and the improvement of treatment group was significantly better than that of the control group ( P <0.05).The TRAb level in the treatment group was significantly lower than that before treatment ( P <0.05).There was no significant difference between the control group and the treatment group ( P >0.05).The TRAb level in the treatment group was higher than that before treatment.There was a significant decrease ( P <0.05).Results:Treatment of initial Graves disease with Yangyin Qingre decoction combined with methimazole can quickly relieve clinical symptoms,improve thyroid function,reduce TRAb levels,reduce immune remission time,reduce patient suffering,and have low adverse reactions.
