经耳后入路内镜辅助下第二鳃裂囊肿切除术

【摘要】 目的 探讨经耳后入路内镜辅助下第二鳃裂囊肿切除术的手术方法,评价其可行性、有效性及安全性。方法 从2008年11月至2011年8月间,6例患者进行了经耳后入路内镜辅助下第二鳃裂囊肿切除术,其中男性2例,女性4例,平均年龄20.6岁(16~33岁)。结果 所有患者均接受完成经耳后入路内镜辅助下第二鳃裂囊肿切除术,无中转经颈入路手术病例。平均手术时间70分钟(60~110分钟),平均手术出血量10 mL(5~15 mL)。无耳大神经或面神经损伤等并发症。平均随访时间为36个月(22~51个月),无复发病例。术后美观效果满意。结论 经耳后入路内镜辅助下第二鳃裂囊肿切除术具有安全、可行、有效,暴露充分,解剖清楚,美容效果好。它可考虑作为此类疾病的新的手术方法。

Endoscope-assisted excision of second branchial cleft cysts via retroauricular approach

Objective To investigate the surgical method of endoscopic-assisted excision of second branchial cleftcysts via retroauricular approach and evaluate its feasibility effectiveness and safety.Methods From November 2008 to February 2011,six patients with second branchial cleft cysts accepted endoscope-assisted excision via retroauricular hairline approach.Results All operations were successfully performed using the endoscope-assisted approach in retroauricular region.There were no conversions of the vianeck approach operations and no postoperative complications were found such as wound infection,bleeding,hematoma and mecrosis of skinflap,injury of great auricular nerve or facial nerve.Average follow-up period was 36 months.（range from：.22-51 months）.No tumor recurrence..was found A good cosmetic results were got in all csses after operation.Conclusion Endoscope-assisted excision of second branchial cleft cysts via retroauricular hairline approach possesses feasibility,safety,effectiveness,adequate exposure of operative feild and a good cosmetic results.So it would be considered as a new surgical method for this disease.
