老年腹股沟斜疝合并小肠破裂6例分析

目的探讨老年腹股沟斜疝合并小肠破裂的原因、临床特点和治疗方式。方法回顾性分析2006年9月至2009年12月徐州市中心医院收治的6例老年腹股沟斜疝合并小肠破裂患者的临床资料。结果 6例中自行粗暴还纳嵌顿性疝导致肠破裂2例,阴囊外伤致肠破裂1例,散步时绊倒发生2例,打篮球跳起投篮时发生1例。手术6例,治愈5例,死亡1例。结论嵌顿性疝粗暴还纳、阴囊外伤是老年腹股沟斜疝合并小肠破裂的常见原因,特殊条件下身体过度后仰亦可诱发肠破裂。肠破裂修补加疝囊颈荷包缝合是治疗腹股沟斜疝合并小肠破裂较好的手术方式。

Treatment of 6 cases of indirect inguinal hernia combined with intestine rupture in elderly patients

Objective To summarize the reason,clinical features and surgical treatment of indirect inguinal hernia combined with intestine rupture in elderly patients.Methods The clinical features of 6 cases of indirect inguinal hernia combined with intestine rupture underwent surgical treatment from September 2006 to December 2009 were analyzed retrospectively.Results Two cases of intestinal rupture were due to reduction of incarcerated hernia carried roughly by patients,1 case resulting from scrotal trauma,2 cases from stumbling while walking,and 1 case occurred as a result of making a basketball jumpshot.Six of them were operated,five patients recovered and 1 patient died.Conclusion Reduction of incarcerated hernia carried roughly and scrotal trauma are the common causes of inguinal hernia associated with intestinal rupture in elderly patients.It may be induced under special instances such as bending over backwards too hard.It will be a better operation choice for treating inguinal hernia combined with intestinal rupture to repair intestinal rupture and close the neck of the hernia sack with a purse-string suture.
