Carpal tunnel release:comparison of the treatment outcome between Okutsu method and open procedure

Objective To compare the treatment outcome between Okutsu method and routine surgical procedure for carpal tunnel release. Methods Endoscopic carpal tunnel release (ECTR) was done in 44 sides of 40 patients with carpal tunnel syndrome. Another 44 sides of 40 patients were treated by open carpal tunnel release (OCTR). The patients were divided into three types according to Hamada's classification before the operation. The patients were followed at 1 month, 3 months, and 12 months postoperatively. Functional assessment was made according to the criteria described by Kelly. Results Functional recovery of the ECTR group was slightly slower than that of the OCTR group 1 month after the operation. Three months postoperatively however, the functional results of the two groups were the same. Conclusions Though the operation approach of ECTR is different from that of OCTR, the result is similar. Compared with OCTR, ECTR has the following advantages: smaller incision and less tissue damage, local anaesthesia, and no tourniquet needed, and no postoperative immobilization.
