隐神经伴行血管蒂岛状皮瓣蒂部成分对皮瓣血运的影响

摘    要:目的 研究皮神经伴行血管蒂岛状皮瓣蒂部各成分对皮瓣血运的影响。方法 16只豚鼠,应用活体超大面积隐神经伴行血管蒂岛状皮瓣切取后美兰灌注法,平均动脉压灌注10分钟,观测染色面积和着色度,根据正交设计原理对蒂部成分进行了五因素、二水平L16^215正交实验和分析。结果 隐神经伴行动脉、静脉及其交互作用对染色面积的影响有显著性意义:皮肤、深筋膜、皮神经、皮神经营养血管网的2~5部分对染色面积的影响无显著意义。结论 隐神经伴行血管及其皮支是该皮瓣的营养血管,皮肤、深筋膜、皮神经、皮神经营养血管内的2~5部分对皮瓣血运的影响无显著意义,

Abstract:objective To study the influence of the multiple pedicle compositions on the blood flow of the island flap pedicled with neurocutaneous colateral vessels. Methods On the basis of L16215 orthogonal experiment design and the method of the supper-sized saphenous nerve collateral vessels axial island flap professed with methylend blue solution, the pedicle compositions were analysed at the average blood pressure and in 10 minutes by measuring the perpused area and color degree. Results The influence of the collateral artery, vein and the action of themselves is very significant, but the skin, fascia, cutaneous nerve and the parts 2 to 5 of the neurocutaneous nutrient vascular network in and around the nerve is no significant. Conclusion The colateral vessels and their subcutaneous branches are the nutrient blood vessels of the flap. The action of the skin, fascia, cutaneous nerve and the parts 2 to 5 of the neurocutaneous nutrient vasular network in the pedicle on the flap circulation is no significant.
