矫正拇掌指关节过伸畸形的新术式

目的为矫正拇掌关节过伸畸形寻求一种行之有效且操作简便的手术方式。方法采用近端切断拇短伸肌腱,保护止点周围的完整,在拇掌指关节掌侧拇长屈肌腱鞘浅面“8”字交叉,最后靠近拇内收肌止点与拇内收肌腱缝合。结果治疗34例拇掌指关节过伸畸形病人,其中类风湿病15例,掌板损伤10例,尺神经损伤9例,平均随访6年,所有过伸畸形完全矫正,未再复发。结论操作简便,疗效佳,有临床应用价值。

A new operative method to correct hyperextension deformity of metacarpophalangeal joint of thumb

ve To find an effective but simple operation pattern to correct hyperextension deformity of thumb's metacarpophalangeal joint. Methods The tendon of the extensor pollicis brevis was severed at proximal end, which insertion was left undisturbed, and was crossed at superficies of tendinous sheath of long flexor muscle of thumb volarwards to metacarpal joint in the pattern of figure of-eight, and finally be sutured with the adductor pollicis closed to its insertion. Results Thirty-four patients who had hyperextension deformities of the metacarpophalangeal joint of the thumb were treated by reinforcement of the volar capsule of the joint using the tendon of the extensor pollicis brevis. Fifteen patients were due to rheumatoid, ten were due to the lesion of the volar plate, nine had injury of ulnar nerve. The patients were followed for an average of 6 years. All deformities were corrected. No case relapsed. Conclusions Clinically it had good effect with little operative difficulty. This would give a bright prospect to correction of hyperextension deformity of the metacarpophalangeal joint of the thumb in the future.
