普通外科手术医师切口感染专率调查

摘    要:目的调查某院普通外科手术医师切口感染专率,探讨控制手术切口感染对策.方法采用前瞻性调查方法,以切口类型、手术持续时间、手术类型、患者健康状况4项危险因素作为危险指数,对2004年2月1日-2005年1月31日该科手术患者切口感染率进行比较.结果调查期间手术切口感染率为3.78%.非清洁手术切口感染率为5.77%,高于清洁手术切口感染率0.39%(P〈.001);手术时间〉 h切口感染率为10.53%,高于手术时间〈 h切口感染率1.20%(P〈.001);急诊手术切口感染率为8.03%,明显高于择期手术切口感染率2.72(P〈.001);危险指数增高,切口感染率随之升高(P〈.001).结论开展手术医师切口感染专率调查,针对危险因素进行监测与控制,可降低手术后切口感染率.

Abstract:Objective To investigate the surgeon-specific incisional wound infection(IWI) rate in the general surgical department of a hospital,and to evaluate the strategies for the control of IWI.Methods Four risk factors(type of incision,duration of operation,type of operation,the state of patients' health) were chosen as risk indexes,IWI rates in patients who were hospitalized from February 1,2004 to January 31,2005 in a general surgical department were compared and analysed prospectively.Results IWI rate was 3.78%,unclean IWI rate was 5.77%,which was higher than that of clean IWI rate(0.39%)(P<(0.001));IWI rate in patients whose duration of operation >2 hours was(10.53%),which was higher than that((1.20%)) in patients whose duration of operation <2 hours(P<(0.001));IWI rate in emergency operation was(8.03%),which was obviously higher than that((2.72%)) of selective operation(P<(0.001));the IWI rate increased with the rising of risk indexes(P<(0.001)).Conclusion The investigation of surgeon-specific surgical IWI rate aiming at monitoring and controlling risk factors is benefit to the reduction of postoperative IWI rate.
