联合检测血细胞因子在小儿肺炎的临床意义

目的探讨不同程度小儿肺炎时,白细胞（WBC）、中性粒细胞弹性蛋白酶（NE）、红细胞沉降率（ESR）、血清C反应蛋白（CRP）和血清心肌肌钙蛋白Ⅰ（cTn-Ⅰ）的变化与临床意义。方法 2008年1月至2010年12月我科收治肺炎患儿80例,根据病情轻重将患儿分为重症肺炎（52例）和非重症肺炎（28例）,所有患儿治疗一周前后进行上述指标检测并进行对比。结果①重症组患儿WBC、NE、ESR、CRP和cTn-Ⅰ水平明显高于非重症组患儿,两组之间的差异有统计学意义（P〈0.05）;②治疗后,两组患儿的WBC、NE、ESR和CRP、cTn-Ⅰ水平较治疗前明显下降（P〈0.05）;③两组治疗后比较,cTn-Ⅰ差异无统计学意义。结论联合检测WBC、NE、ESR、CRP和cTn-Ⅰ等多项指标,可很好地反映病情轻重变化。重症肺炎患儿尤其合并心衰者,cTn-Ⅰ明显升高,联合检测cTn-Ⅰ能更准确地进行病情判断,其下降预示病情改善。

Clinical analysis of 52 children with severe Pneumonia

Objective To investigate the changes in the levels of white blood cell(WBC),neutrophil elastase(NE),Erythrocyte sedimentation rate(ESR),C-reactive protein(CRP) and cardic troponinⅠ(cTn-Ⅰ) based on different degrees of pneumonia in children and their clinical meaning.Methods 88 children with pneumonia admitted in our hospital from Jan.2008 to Dec.2010 were assigned into two groups based on the degree of the disease:group A(52 cases,with severe pneumonia) and group B(28 cases,with non-severe pneumonia).Before treatment and one week after the treatment,the levels of WBC,NE,ESR,CRP and cTn-Ⅰwere detected.Results The levels of WBC,NE,ESR,CRP and cTn-Ⅰin group A are significantly higher than that of group B(P<0.05);All the values in the two groups decreased obviously after treatment(P<0.05).The levels of cTn-Ⅰ after treatment between the two groups showed no statistically significant difference.Conclusion The levels of WBC,NE,ESR,CRP and cTn-Ⅰ can reflected the severity of the disease.For children suffered from severe pneumonia,especially those complicated with heart failure,the examination of levels of cTn-Ⅰ can help the diagnosis of decrease.
