AECOPD患者病情严重程度与甲状腺激素水平、D-二聚体、CRP的相关性研究

目的 探讨慢性阻塞性肺疾病急性加重期(AECOPD)患者病情严重程度与甲状腺激素测定结果、D-二聚体、CRP相关性及其临床价值.方法 选择2010年1月～2012年1月某院住院确诊AECOPD患者60例(有呼吸衰竭的30例,无呼吸衰竭30例)与30例对照组(非AECOPD患者)的血浆甲状腺激素定量结果、D-二聚体、CRP进行比较.结果 AECOPD患者(呼吸衰竭组)血浆甲状腺激素定量结果、D-二聚体、CRP明显高于AECOPD(无呼吸衰竭组),差异有统计学意义(P＜0.05),也明显高于对照组,差异有统计学意义(P＜ 0.05),COPD(无呼吸衰竭组)也高于对照组,差异有统计学意义(P＜0.05).结论 AECOPD患者血浆甲状腺激素水平明显降低,D-二聚体及CRP明显升高,并且其降低及升高程度与AECOPD严重程度相关,可作为AECOPD病情严重程度的判定指标之一.

The correlation analysis of the severity of the patients with AECOPD disease and thyroid hormone level,D-dimer and CRP

OBJECTIVE To investigate the disease severity of chronic obstructive pulmonary disease with acute exacerbation(AECOPD) in patients and thyroid hormone,D-dimer,CRP and its clinical value.METHODS Selected the 60 cases diagnosed with AECOPD in hospital from January 2010 to January 2012(30 cases with respiratory failure,30 cases without respiratory failure) and the 30 cases in the control group(patients without AECOPD),compared their thyroid hormone,D-dimer and CRP.RESULTS The plasma thyroid hormone,D-dimer and CRP of patients with AECOPD(respiratory failure group) were significantly higher than those with AECOPD(non-respiratory failure group),the difference was statistically significant(P﹤ 0.05) and it was also significantly higher than the control group(patients without AECOPD),the difference was statistically significant(P﹤0.05),with COPD(non-respiratory failure group) also higher than the controls,the difference was statistically significant(P﹤0.05).CONCLUSION The plasma thyroid hormone level in AECOPD patients is significantly decreased,but D-dimer and CRP are significantly increased,which have a correlation with AECOPD severity,can be used as one of the indicators in predicating AECOPD severity.
