糖皮质激素治疗对过敏性紫癜反复及远期肾损害的影响

目的 研究糖皮质激素治疗对过敏性紫癜复发及远期肾损害的作用.方法 2008年1月～2010年6月期间收集某院儿科过敏性紫癜患者128例,采用随机数字法进行随机分组,随机均分为观察组与对照组2组,观察组在常规治疗基础上予糖皮质激素治疗,对照组予常规治疗,比较2组的紫癜复发情况、随访肾损害情况.对各指标率的变化采用组间t检验,计数资料采用组间x2检验.结果 随＝期间观察组过敏性紫癜复发率45.2％ (28/62),对照组复发率80.0％(48/60),二者相比较差异有统计学意义,x2=7.878,P=0.005.观察组过敏性紫癜肾损害发生率32.3％ (20/62),对照组肾损害发生率73.3％ (44/60),二者相比较差异有统计学意义,x2=10.314,P=0.001.结论 糖皮质激素治疗能够减少HSP复发次数,并能够减少远期肾损害,需临床进一步扩大样本验证.

Glucocorticoid treatment on allergic purpura repeatedly and long-term renal damage in rats

OBJECTIVE To study the effects of glucocorticoid therapy on Henoch-Schonlein purpura recurrence and long-term renal damage.METHODS From 2008 to January to June 2010,128 cases with allergic purpura were collected in our hospital by using the random number collection,and they were randomly divided into the observation group and the control group.The observation group on the basis of routine treatment with corticosteroid therapy,and the control group received routine treatment.The purpura relapse and renal damage were compared.The index rate changes between groups was analyzed by t test,and count data was analyzed by χ 2 test.RESULTS The allergic purpura relapse rate in the observation group was 45.2%(28 /62),and was 80%(48 /60) in the control group.There was no significant difference between the two groups,χ 2 = 7.878,P﹤0.05.The renal damage rate in the observation group was 32.3%(20 /62),and was 73.3%(44 /60) in the control group,and there was significance difference between them,χ 2 = 10.314,P﹤0.05.CONCLUSION The glucocorticoid therapy can reduce the HSP relapses,and can reduce the need for clinical long term renal damage,further expansion of the sample verification.
