摘    要:目的 对比封闭负压辅助闭合(vacuum-assisted closure,VAC)技术与传统换药方法对糖尿病足溃疡的临床疗效.方法 对46例糖尿病足溃疡患者随机分为VAC治疗组(23例)和对照组(23例),VAC治疗组采用局部清创、控制血糖、抗感染等常规治疗,在其分泌物细菌培养阴性前提下,加行VAC治疗,待创面肉芽填充丰满后行常规换药至伤口愈合.对照组采用常规换药技术,常规清创后,定期换药至伤口愈合.结果 VAC治疗1~3个疗程,患者创面肉芽组织生长新鲜、红润,无感染、水肿、渗出,创面愈合时间( 50.4±5.3)d.对照组创面愈合时间(81.1±5.9)d.两组对比差异有统计学意义(P<0.01).结论 VAC治疗糖尿病足溃疡可改善其局部血运,减轻局部水肿,促进创面肉芽组织生长,缩短创面愈合时间.

Abstract:OBJECTIVE To contrast the clinical curative effects of vacuum-assisted closure(VAC)and traditional method of interference in diabetic foot ulcers.METHODS 46 patients with diabetes foot ulcer VAC patients were randomly divided into the treatment group(23 cases)and the control group(23 cases).VAC treatment group used conventional treatments such as local debridement,blood sugar control and anti-infection.On the premise of negative bacteria cultivation in secretion,applied VAC treatment,when granulation filling in wound,gave conventional medicine change.The control group applied with conventional technology,after conventional debridement,change dressings regularly to wound healing.RESULTS VAC for 1-3 courses of treatment,patients’ wound granulation tissue grew fresh and ruddy,without infection,edema and exudation,time of wound healing was(50.4 ± 5.3)d.The time of wound healing was(81.1 ± 5.9)d in the control group.There was a significant difference in two groups(P﹤0.01).CONCLUSION VAC treatment in diabetes foot ulcer can improve local blood supply,reduce local edema,promote wound granulation tissue growth,shorten the wound healing time.
