动脉瘤性蛛网膜下腔出血术后低钠血症相关危险因素分析及临床特点

目的 探讨动脉瘤性蛛网膜下腔出血术后低钠血症发生的相关危险因素,观察动脉瘤性蛛网膜下腔出血术后低钠血症的临床特点.方法 回顾性分析2004年3月～2011年4月126例在某院收治的动脉瘤性蛛网膜下腔出血患者的临床资料,分析术后低钠血症的相关危险因素及临床特点.结果 126例患者中有35例术后并发低钠血症,占27.78％.将性别、年龄、术前Hunt-Hess分级、动脉瘤部位、手术时机等危险因素引入二项分类Logistic回归分析模型之中,性别、手术时机等与低钠血症发生无关,差异无统计学意义(P＞0.05),年龄、动脉瘤部位、术前Hunt-Hess分级等与低钠血症发生有关,差异有统计学意义(P＜0.05).结论 年龄、动脉瘤部位、术前Hunt-Hess分级等与低钠血症发生有关,50～60岁之间,前交通动脉瘤,术前Hunt-Hess分级级别越高,其术后并发低钠血症的可能性越大.

Aneurysmal subarachnoid hemorrhage postoperative hyponatremia risk factors and clinical features

OBJECTIVE To investigate the risk factors associated with aneurysmal subarachnoid hemorrhage,postoperative hyponatremia occurred,and the clinical features observed after aneurysmal subarachnoid hemorrhage hyponatremia.METHODS Retrospective analysis was used for the clinical data of 126 cases in our hospital with aneurysmal subarachnoid hemorrhage in March 2004 to April 2011.We analyzed the risk factors associated with postoperative hyponatremia and clinical characteristics.RESULTS Among 126 patients,35 cases were postoperative hyponatremia,accounting for 27.78%.Risk factors included gender,age,preoperative Hunt-Hess grade,aneurysm site and the timing of surgery.Logistic regression analysis model showed gender and timing of surgery had no association with hyponatremia(P﹥0.05) Age and aneurysm site had association with preoperative Hunt-Hess grade hyponatremia(P﹤0.05).CONCLUSION Age,aneurysm site,preoperative Hunt-Hess grade had association with hyponatremia.Ages of 50-60,the anterior communicating artery aneurysm,Hunt-Hess grade level the higher the preoperative and postoperative are the risk factors for hyponatremia.
