无创宫腔镜检查在门诊的临床应用

目的评价无创宫腔镜检查技术在门诊应用的临床效果。方法 2011年6月,笔者所在医院门诊实施宫腔镜检查术155例,其中无创宫腔镜检查72例,常规宫腔镜检查83例,记录检查时间,并以McGill疼痛问卷来评价患者手术时的疼痛程度及舒适度。用5分法评价宫腔镜图像质量。结果两组术后病理检查符合率无创组为94.44%和84.34%,差异显著(P=0.045)。无创组手术时间明显短于常规组(P<0.01)。McGill不适评分、恐惧评分、折磨人评分、疲倦评分及疼痛评分无创组与常规组均差异有统计学意义(P<0.01)。宫腔镜图像评分无创组明显优于常规组。无创组4例因疼痛剧烈,要求行静脉麻醉,常规组14例行静脉麻醉,差异有统计学意义(P<0.05)。结论无创宫腔镜检查明显减少患者的疼痛和不适感,无需麻醉,值得推广。

Clinical application of non-invasive hysteroscopy in outpatient clinic

Objective To evaluate the clinical effect of non-invasive hysteroscopy in the outpatient clinic.Methods 155 outpatients were given diagnostic hysteroscopy in June 2011.72 cases were given non-invasive hysteroscopy,83 cases were given routine hysteroscopy.The examination period were recorded.The comfort and pain were evaluated by McGill Pain Questionnaire.The quality of visualization of the uterine cavity was recorded using a 5-point scale(1=excellent to 5 =insufficient).Results 94.44% diagnoses by non-invasive hysteroscopy were in keeping with the diagnoses by pathology,which was 84.34% by routine hysteroscopy.There was statistically significant between the two groups(P < 0.01).The average examination period in non-invasive hysteroscopy group was significant shorter than the period in routine hysteroscopy group.Discomfort score,terror score,torture score,tiredness score and pain score was significant lower than those in routine group.4 patients feel acute pain during examination in non-invasive group and 14 cases in routine group,which is significant(P < 0.05).These patients were examined under intravenous anesthesia.Optical properties in non-invasive group were judged to be far superior to those in routine group(P < 0.05).Conclusion The patients feel less pain and discomfort,and needn’t anesthesia during non-invasive hysteroscopy.So non-invasive hysteroscopy is worth a wide application.
