经颅磁刺激疗法联合尼莫地平对脑梗死后血管性痴呆的疗效观察

目的观察应用尼莫地平与经颅磁刺激(TMS)联合治疗脑梗死后血管性痴呆的疗效。方法急性脑梗死后血管性痴呆患者114例,分为对照组35例、尼莫地平组40例、尼莫地平联合TMS组39例。对照组给予常规药物治疗,尼莫地平组在常规药物治疗基础上加用尼莫地平片30 mg口服,1次/d,14 d为1个疗程,共进行2个疗程;尼莫地平联合TMS给予常规药物、尼莫地平及TMS疗法,每次30 min,每天2次,4周为1个疗程。治疗前后进行简易精神状态检查法(MMSE)、蒙特利尔认知评估量表(MoCA)及修改后长谷川痴呆量表(HDS-R)评定。结果经4周治疗,尼莫地平联合TMS组在MMSE、MoCA、HDS-R评分均高于治疗前(P<0.05),总体有效率为87.18%,其疗效优于对照组及单独应用尼莫地平组(P<0.05)。结论尼莫地平联合TMS综合治疗能显著改善血管性痴呆患者的临床症状。

Effect of Transcranial Magnetic Stimulation in combination with nimodipine on vascular dementia after cerebral infarction

Objective to investigate the effect of Transcranial Magnetic Stimulation(TMS) in combination with Nimodipine on Vascular Dementia after cerebral infarction. Methods 114 patients with Vascular Dementia after cerebral infarction were assigned to the control group 35 cases, Nimodipine group 40 cases, and Nimodipine in combination with TMS group 39 cases randomly. Each patient was given conventional drugs therapy. The control group only were given conventional drugs.Nimodipine group had add Nimodipine Tablets,30mg,qd,four weeks of course of treatment. TMS group had add low frequency electromagenetic therapy. Nimodipine in combination with TMS therapy group received Transcranial Magnetic Stimulation treatment, two times a day for 30 minutes, 28 days of course of treatment. The three groups were assessde with mini-mental state examination(MMSE), Montreal Cognitive Assessment Scale (MoCA)and Hasegawa Dementia scale(HDS-R) before and after treatment.Result After four weeks,MMSE scores ,MoCA scores and HDS-R scores of Nimodipine in combination with TMS therapy increased in comparison with the other two groups(P<0.05).
