高低频率结合喷射通气的动物实验及临床观察

摘    要:本文应用高频喷射振荡(HFJO)、低频喷射通气(LFJV)以及高频喷射振荡叠加低频喷射通气(HFJO+LFJV)三种通气方式,观察对无自主呼吸犬的氧合及CO_2排出的效果。三种通气方式都能维持较好的氧合,但HFJO+LFJV能有效地排出CO_2,防止CO_2潴留。临床应用观察到,对于快速静脉诱导的无自主呼吸的麻醉病人,和HFJO相比,HFJO+LFJV在气道内平均压无明显增加的情况下,可使病人体内CO_2有效地排出,维持动脉血pH在正常范围。我们认为,在气道开放状态下HFJO+LFJV能增强CO_2的排出,维持正常的气体交换,不产生对循环的不良影响。

Abstract:High frequency jet oscillation(HFJO),low frequency jet ventilation(LFJV)and HFJO com-bined with LFJV(HFJO LFJV)were used to ob-serve their effects on oxygenation and CO_2 elimina-tion of dogs without autonomic breath. The resultsrevealed that all of the three Kinds of ventilationshave efficient oxygenation, but HFJO LFJV couldeffectively eliminate CO_2, preventing the retention ofCO_2. Clinical observation revealed that, for the anes-thetized patients without autonomic breath,com-pared with HFJ0, HFJO LFJV effectively eliminat-ed CO_2 and maintained the normal pH, while themean airway pressures were not increased. We con-clude that, with the airway open, HFJO LFJVcould enhance the elimination of CO_2, maintain thenormal gases exchange and don't interfere with bloodcirculation.
