乳晕分层切口在乳腺良性肿块切除术中的应用

目的探讨乳晕分层切口在乳腺良性肿块切除术中的临床应用价值。方法对2009年1月～2010年8月在成都市妇女儿童中心医院乳腺科住院的126例乳腺良性肿块住院患者进行乳晕分层切口切除肿块,对切除肿块后所留的残腔采用不缝合技术,通过3个月～3年随访,评价对乳腺良性肿块的治疗效果。结果 126例乳腺良性肿块患者均成功切除（100%）,单个病灶长径为0.5～6cm,病灶数量为1～12个,平均手术时间37min（5～162min）,乳房切口疤痕微细,保持乳房外形,硬块感率21.4%,硬块感时间平均30天（20～40天）,患者满意98.8%。患者术后无严重并发症。结论在乳腺良性肿块切除中应用乳晕分层切口及残腔无缝合技术操作简单、美观、安全可行、无严重并发症,具有临床推广应用价值。

Application of mammary areola layered incision in resection of mammary gland benign masses

Objective To explore the clinical application value of mammary areola layered incision’s application in resection of mammary gland benign masses.Methods 126 patients with mammary gland benign masses were treated with mammary areola layered incision to resect masses.Non-suture technique was taken to suture residual cavities caused by resection of masses.And through three months to three years of follow-up visit,the therapeutic efficacy to mammary gland benign masses would be evaluated.Results 126 mammary gland patients were successfully resected.The major diameter of each single nidus is from 0.5 centimeter to 6 centimeters,the number of niduses was one to twelve,and the surgery time was 37 minutes(from 5 to 162 minutes).The scar of breast incision was small and not easy to be seen,which kept the good shape of breast.The rate of hard lump sense was 21.4%,the average time of hard lump sense was 30 days(from 20 to 30 days),and the degree of satisfaction of patients was 98.8%.After surgery,no serious complication would happen to patients. Conclusion In the resection of mammary gland benign masses,it is simple to operate mammary areola layered incision and non-suture technique for residual cavity.Besides,it keeps the good shape of breast,secure and feasible.No serious complication would happen.So it is of clinical promotion and application value.
