摘    要:目的:评价体检筛查结合钼靶X线与超声对乳腺癌的诊断价值,以提高乳腺癌的检出率。方法:选择经手术病理证实的乳腺癌患者19例,对其X线、超声征象及其敏感性进行统计学分析。结果:①检出乳腺癌的敏感性分别为:体检筛查0.34%,钼靶X线68.4%(13/19),高频超声68.4%(13/19),两者比较差异无统计学意义;综合两者影像检查的敏感性为100%(19/19),明显优于单一钼靶X线或超声检查(P〈0.05)。②钼靶X线与超声显示乳腺癌的征象有毛刺肿块、微细钙化、肿瘤血管及多种不典型征象。③钼靶X线对微细钙化检出率为47.4%(9/19),高频超声检出率为26.3%(5/19)。钼靶X线检出钙化型乳腺癌比高频超声敏感(P〈0.05)。④高频超声对乳腺癌血流信号检出率为68.4%(13/19),钼靶X线显示异常血管征为31.6%(6/19)。高频超声检测血流信号明显优于钼靶X线(P〈0.01)。结论:体检筛查是重要的乳癌早期诊断方法,钼靶X线与高频超声作用互补,综合两种影像检查能显著提高乳腺癌的检出率。

Abstract:Objective:To evaluate the combination of medical screening,molybdenum X-ray and ul trasonography in the diagnosi s of breast cancel,in order to improve the detecti on rate of breast cancer.Methods:19 casesof breast.cancer con.rmed by surgery and pathoogy,the signs ofmolybdenum X-ray and ultrasonography and their sensi tivity were analyzedstatisticay.Results:①the sensitivity of detection of breast cancer was as follows:medical screening 0.5%:manmlography x-ray,68.4%(13/19):high-frequency ul trasound,68.4%(13/19).There was rio stati sticsignificances between mammography x-ray and high-frequency ul ttasound insensitivity of detection of breast cancer:the sensi tivi ty could be~raisedto 100%(19/19)by combining the two methods and superior to the singemethod used(P0.05).②Mammography X-ray and ul trasonograph,showssigns of breast cancer had burr mass,micro-calcification,tumor bloodvessels and a variety of atypical signs.③The detection r8te of a.micro-calcification of mammography x-ray was 47.4%(9/19):the detectionrate of a micro-calcification of high-frequency ultrasound was 26.3%(5/19).the mammography x-ray was more sensitive than high-frequencyultrasound in detection rete of the calcified breast cancer(P0.05).④The detection rate of breast cancer's blood flow was 68.4%(13/19)by high-frequency ultrasound;the detect Jon rate of abnormalvessels was31.6%(6/19)by mammography X-ray.High-frequency ultrasound was superiorto mammography X-ray in detection of abnormat bloodflow(P0.01).Conclusion:Medi cal screening is an important method of early diagnosis of breast cancer.The mammography X-ray and high-frequency ultrasound complement each other and combining these two methods could significantly improved the detection rate of breast cancer.
