Abstract:Objective:To investigate the effects of positive and-expiratory pressure(PEEP)on oxygen utilization coefficient(O2UC) in patients with acute respiratory sistress syndrome(ARDS)Methods: We studied 28 ARDS patients with mechanical ventilation,Catherer of central vein was laid.Increment levels of PEEP(0 cmH2O,5,10,15 and 20 cmH2O) were applied sequentially.Arterlial and venous bloods wer taken respectively in order to measure the oxygen metabolism parameters.Calculations of O2UC[O2UC=(SaO2-SvO2)/ SaO2]were carried out respectively.Meanwhile.Normal arterial and venous bloods of 30 cases were collected,and calculated the O2UC as control.Results: Arterial oxygen tension(PaO2) increased significantly(P0.05) at PEEP 5 cmH2O.Oxygen utilization coefficient(O2UC),heart rate(HR) and mean blood pressure(MBP) were not significantly different(P0.05) at PEEP 10 cmH2O.At PEEP 15 cmH2O,O2UC and HR increased significantly(P0.05),but MBP was reduced obviously(P0.05).Conclusion:High PEEP could result in AROS patient oxygen utilization coefficient's further descend,and could not really correct patient organization cell's oxygen deficiency condition.The optimal PEEP should been found,and he blood capacity and effective cardiac output should been appropriately incaeased,which and really improve the patient's hypoxia.
