银杏达莫联合阿替普酶治疗急性高危肺栓塞临床评价

摘    要:目的探讨银杏达莫联合阿替普酶治疗急性高危肺栓塞的疗效及对患者纤溶功能和血小板功能的影响。方法选取医院2016年4月至2018年11月收治的急性高危肺栓塞患者103例,按随机数字表法分为对照组(51例)和研究组(52例)。两组患者入院后均予低分子肝素抗凝、呼吸支持、直接再灌注等常规治疗,并予注射用阿替普酶,研究组患者加用银杏达莫注射液。两组疗程均为14 d。结果研究组总有效率为78.85%,明显高于对照组的60.78%(P<0.05);治疗后,两组患者D-二聚体(D-D)、纤维蛋白原降解产物(FDP)、平均血小板体积(MPV)、血小板分布宽度(PDW)均较治疗前明显降低,且研究组明显低于对照组(P<0.05);对照组与研究组不良反应发生率相当(13.73%比19.23%,P>0.05)。结论银杏达莫联合阿替普酶治疗急性高危肺栓塞疗效确切,可有效改善患者的纤溶功能及血小板功能。

Ginkgo Leaf Extract and Dipyridamole Injection Combined with Alteplase for Acute High-Risk Pulmonary Embolism

Abstract:Objective To investigate the efficacy of ginkgo leaf extract and dipyridamole injection combined with alteplase in the treatment of acute high-risk pulmonary embolism and its effect on fibrinolysis and platelet function.Methods A total of 103 patients with acute high-risk pulmonary embolism in the hospital from April 2016 to November 2018 were selected and divided into the control group(n=51)and the study group(n=52)according to the random number table method.After admission,patients in both groups were treated with low molecular weight heparin,anticoagulation,respiratory support,and direct reperfusion.The control group was given alteplase,and the study group was treated with ginkgo leaf extract and dipyridamole injection on the basis of the control group.Both groups were treated for 14 d.Results The total effective rate of the study group was 78.85%,which was significantly higher than60.78%of the control group(P<0.05).Compared with before treatment,the D-dimer(D-D),fibrinogen degradation product(FDP),mean platelet volume(MPV)and platelet distribution width(PDW)in both groups significantly decreased,and the levels in the study group were significantly lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(13.73%vs.19.23%,P>0.05).Conclusion Ginkgo leaf extract and dipyridamole injection combined with alteplase is effective in the treatment of acute high-risk pulmonary embolism.It can effectively improve the fibrinolytic function and platelet function of the patients.
