氯沙坦和缬沙坦对老年高血压患者疗效的比较

目的　比较氯沙坦和缬沙坦对老年高血压患者的降压疗效和安全性。方法　80例轻-中度老年高血压患者随机选氯沙坦(5 0 mg·d- 1 )或缬沙坦(80 mg·d- 1 ) ,共16周。每4周1次测诊室坐位谷值舒张压、脉率。结果　治疗16周时,氯沙坦组DBP12 .0±0 .7k Pa和缬沙坦组DBP 11.9±0 .5 k Pa,两组治疗前后比较有显著差异(P<0 .0 0 1)。第16周氯沙坦组血尿酸(SU A)从35 7μm ol·L- 1 下降到333μm ol·L- 1 (P<0 .0 5 ) ,缬沙坦组SU A从35 1μm ol·L- 1 升高到36 3μm ol·L- 1 (P>0 .0 5 ) ,两间治疗后比较有显著差异(P<0 .0 0 1)。不良反应发生率无显著差异。结论　氯沙坦与缬沙坦对老年轻中度高血压患者疗效和不良反应发生率无明显差异,氯沙坦可以降低SU A。

Comparison of the therapeutic effect between losartan and valsartan in elderly hypertensive patients

Objective To compare the efficiency and safety of losartan and valsartan on blood pressure in elderly patients with primary hypertension.Methods Eighty elderly patients with mild-moderate hypertension were randomly assigned to receive losartan (50 mg·d -1 ) or valsartan(80 mg·d -1 )for 16 weeks.A trough sitting diastolic blood pressure(DBP) and pulse rate was measured before and 4,8,12,16 weeks after medication.Results The DBP at end of treatment in losartan group and valsartan group were 12.0±0.7 kPa and 11.9±0.5 kPa respectively,compared with the levels before treatment in two group were significanly difference(P<0.001).The mean serum uric acid levels in losartan group decreased from 357 μmol·L -1 before treatment to 333 μmol·L -1 (P<0.05)and that in valsartan group increased from 351 μmol·L -1 to 363 μmol·L -1 (P>0.05),but the levels in the end of treatment between groups was significanly difference(P<0.001).The incidence of adverse effects in two group was did not significanly difference.Conclusion Both losartan and valsartan are similarly effective in reducing blood pressure in elderly patients with mild to moderate hypertension,but losartan can decrease serum uric acid level.
