尺骨撞击综合征的MRI表现

摘    要:目的:探讨MRI在尺骨撞击综合征诊断中的应用价值。方法:对56例尺骨撞击综合征患者的MRI表现进行影像学分析。结果:尺骨撞击综合征MRI表现为:(1)Yl骨尺侧面、三角骨桡侧面及尺骨关节软骨不同程度损伤、退变,关节软骨形态出现不规则、变薄或消失,软骨内可出现小囊性病灶或纤维化。(2)软骨下骨髓可出现出血、水肿;也可出现囊性坏死灶;软骨下骨质可出现增生、硬化。(3)三角纤维软骨盘不同程度损伤、退变:软骨盘内出现小片水肿或变性;软骨盘内出现水平撕裂;软骨盘内出现累及关节面缘的撕裂;软骨盘中央穿孔;软骨盘桡或尺侧附着端撕裂;软骨盘整体形态不规则、变薄或消失。结论:MRI可较好的显示尺骨撞击综合征所致的月骨尺侧面、三角骨桡侧面及尺骨关节软骨、软骨下骨及三角纤维软骨盘不同程度损伤、退变,通过影像学分析,可了解发病机制及早期诊断。

Abstract:Objective: To discuss the application value of MRI in diagnosis of ulnar impaction syndrome. Methods: The MRI characters of 56 ulnar impaction syndrome cases were analyzed. Results: the MRI manifestation of ulnar impaction syndrome: (1)different degree damages and retrogression were observed in ulnar aspect of lunate, radius aspect of traguetral and arthrodial cartilage the irregulation, attenuation or disappearance in the formation of arthrodial cartilage of ulnar. There were and the small cystic lesions or fibrosis in the cartilaginous opereulum. (2) Bleeding, swelling or cystic necrotic foci were observed in subchondral bone marrow; proliferation and sclerosis were observed in subchondral bone tissue. (3)Different degree damages and retrogression were observed in triangular fibrocartilage disc:There were the small swelling or degeneration, horizontal tear and tear that extent to the joins surface in the fibrocartilage disc. Irregulation, attenuation or disappearance of the fibrocartilage disc were observed. Conclusion: MRI can clearly show the damages and retrogression with different degrees in ulnar aspect of lunate, radius aspect of traguetral and arthrodial cartilage of ulnar. We can understand the pathogenesis and early diagnose by analyzing imaging characters.
