晚期妊娠合并卵巢子宫内膜异位囊肿(附8例病例分析)

8例经手术及病理证实的晚期妊娠伴发子宫内膜异位囊肿的患者,平均年龄29岁,均无不孕史,其中原发痛经7例。孕期中3例合并中央性前置胎盘。剖宫术中见病变主要累及单侧附件,属R-AFS分期Ⅲ-Ⅳ期。1例仅吸出囊液,余行单附件切除或囊肿剥除。术后随访1a以上者6例,0.5a内2例。7例痛经者6例消失;3例再孕;1例原对侧正常附件处有小囊肿在观察中。对妊娠合并卵巢子宫内膜异位囊肿的临床表现、并发症及诊断和处理进行了讨论。

THIRD TRIMESTER OF PREGNANCY ASSOCIATED WITH OVARIAN CYST OF ENDOMETRIOSIS (8 CASES REPORT)

8 pregnant women in third trimester associated with ovarian cyst of endometriosis were analyzed. The mean age was 29. Primary dysmenorrhea was complained in 7 cases. 3 of them were associated with central placenta previa. Unilateral ovarian chocolate cyst with almost normal adnexa on opposite was found in all the eight cases. Unilateral adnexectomy or enucleation of chocolate cyst were performed in 7 cases and cleaning out chocolate material in 1 case. 6 patients have been followed up for more than 1 year, 3 of them, over 3 years, only 2 cases less than 6 months. Dysmenorrhea disappeared in 6/7 cases. 3 patients have another pregnancy. In 1 case, ovarian cyst (diameter less than 5 cm) was found on the opposite side. Clinical manifestation, complications, diagnostic problems and the therapeutic principle for ovarian cyst of endometriosis associated with pregnancy were discussed.
