乳腺肿块的钼靶X线鉴别诊断

目的分析乳腺良恶性肿块的X线表现特征,以提高钼靶X线鉴别诊断水平。方法通过对经术后病理检查证实的66例75个乳腺肿块钼靶X线表现进行回顾性分析,其中恶性肿块41例(45个乳腺肿块),良性肿块25例(30个乳腺肿块),5例为双侧乳腺肿块。结果乳腺肿块具有以下表现特点中的两个或两个以上,应高度提示恶性肿块。①肿块边缘不规则,分叶状或带毛刺状的肿块;②肿块内或邻近有成簇微细钙化灶;③肿块密度不均匀;④肿块周围见不规则透明晕及局部结构紊乱、扭曲。其中是带毛刺状的肿块和肿块内或邻近有成簇微细钙化灶是诊断乳腺癌特异性X线征象。而乳腺良性肿块边缘清楚、光滑,密度均匀或不均匀,可见粗大钙化或脂肪低密度影,肿块周围均匀的宽窄一致的透明晕。本组良恶性乳腺肿块钼靶X线诊断准确率分别为86.8%和92.1%。结论乳腺钼靶X线检查在乳腺肿块良恶性鉴别中具有重要意义。
