经椎弓根固定矫正脊柱侧凸失误和并发症的预防及处理

采用自行研制的经椎弓根多钉固定撑压系统治疗脊柱侧凸５０例取得显著疗效。报告了术中操作失误和术后并发症的预防和处理。术中操作失误主要是：椎弓根定位不准（５．３％）、螺钉过长、折弯。预防的关键是椎弓根螺钉的准确定位及选择合适的螺钉。术后并发症主要是断棒、脱钉及肠系膜上动脉综合征。对上述并发症提出了处理措施。

Prevention and management of faults and complication of surgical treatment of scoliosis by fixation of the vertebral pedicle

Significant effect has been obtained in 50 cases with scoliosis by the system by ourselves developed of multi-screw fixed distractor-compressor via the vertebral pedicle from Dec.1989 to Dec.'1992.The prevention and management of the faults during the procedure and complications following the operation were reported. The faults mainly included 13 imprecise drilling points at the pedicle in 245 screws (5. 3%), 2 excessive length of screws, 2 bending screws, 1 puncture into meninges and pleura, and 1 injury of nerve root. The key to the prevention of above-mentioned faults is precise location and proper choice of screw. Post-opera tive complications were 3 cases of broken rod, 2 lossening of screws and 1 superior mesenteric artery syndrome.The management of the complications was also suggested.
