| 摘 要： | 目的研究高龄老年慢性心力衰竭(心衰)患者N末端B型脑钠肽前体(NT-proBNP)的变化及静脉注射冻干重组人脑利钠肽(rhBNP)治疗心衰患者的临床效果。方法检测30例慢性心衰患者NT-proBNP,同时观察应用冻干rhBNP的治疗效果及相关指标,NT-proBNP、心率、血压、心功能分级、射血分数、肌酐及尿量的变化。结果心衰组NT-proBNP明显升高,应用冻干rhBNP治疗后心功能大部分得到改善,总有效率为66.7%,其中显效11例,有效9例,无效3例,加重2例,死亡5例,其中因心衰死亡2例。心衰组治疗后NT-proBNP下降,射血分数上升,心率下降,血压及肌酐无明显变化,但血压有下降趋向;尿量有减少趋向,排除2例因慢性肾功能不全行血液透析的患者后,治疗后尿量增加。结论 NT-proBNP在高龄老年慢性心衰患者中水平升高,冻干rhBNP治疗慢性心衰有效,且长期应用也比较安全。
