超声弹性评分在甲状腺实质性结节良恶性鉴别诊断中的应用

目的评价弹性评分在甲状腺实质性结节良、恶性鉴别中的应用价值。方法对2009年4月至2009年7月72例(共102个结节)甲状腺实质性结节患者术前进行弹性成像超声检查,获得弹性评分结果,以手术病理为金标准,分析弹性评分在实质性结节良恶性组中的差异,并运用ROC曲线评价弹性评分在甲状腺实质性结节良恶性鉴别诊断中的价值,总结常见甲状腺实质性结节的弹性成像表现。结果良恶性甲状腺结节(29个恶性结节与73个良性结节)的弹性评分的分布,差异有统计学意义(χ2=36.064,P<0.05);弹性评分法诊断甲状腺实质性结节良、恶性敏感度为93.1%(27/29),特异度为72.6%(53/73),阳性预测值为57.4%(27/47),阴性预测值为96.4%(53/55)。阳性似然比为3.39,阴性似然比为0.09,准确性为78.4%(80/102),ROC曲线下面积为0.822。结论甲状腺实时弹性成像评分是评价甲状腺结节的良好指标,为超声鉴别诊断良恶性甲状腺实质性结节提供有效的判别指标。

Differential diagnosis of solid thyroid nodules with elastic strain scores on ultrasonography
