EGFR、CD44v6及PGP在大肠癌中的表达及意义

目的探讨大肠癌中表皮生长因子（EGFR）、吞噬细胞黏附分子（CD44v6）和多药耐药基因蛋白（PGP）表达与病理组织学特征及预后的关系.方法对220例大肠癌石蜡标本进行重新切片,分别采用EGFR、CD44v6和PGP单克隆抗体进行免疫组化标记（S-P法）.并对不同大肠癌组织学分型、分化程度、浸润深度、临床分期（Dukes）及预后组的染色结果进行比较.结果不同大肠癌组织学分型、分化程度、浸润深度组EGFR、CD44v6和PGP阳表达率的差异均无统计学意义（均P＞0.05）;而肿瘤伴淋巴结转移组及Dukes C期组,EGFR、CD44v6阳性表达率高于无淋巴结转移组及Dukes A、B期组（均P＜0.05）.3、5年随访资料表明,死亡组EGFR、CD44v6和PGP阳性表达率高于生存组（均P＜0.01）.结论EGFR、CD44v6和PGP的阳性表达提示直肠癌患者预后较差,联合检测该三项指标,对判断大肠癌预后有很大帮助.

Expression of EGFR, CD44v6 and PGP in colorectal carcinoma and its clinical significance

Objective To investigate the expression of EFGR, CD44v6 and PGP in colorectal carcinoma and its clinicopathological relation. Methods Expression of EFGR, CD44v6 and PGP was detected with corresponding monoclonal antibodies by immunohistochemical S-P staining in 220 specimens of colorectal carcinoma. The immunohistochemical results were compared with degree of differentiation, tumor invasion, lymphnode metastasis, Dukes stages and prognosis. Results There was no significant correlation of these protein expression with tumor size, histological grades, degree of differentiation, and tumor invasion (P>0.05). However, the positive rates of EFGR, CD44v6 in cases with lymphnode metastasis and Dukes C were much higher than those without lymphnode metastasis and Dukes A and B (P<0.05). The 3 and 5 year follow-up data showed that the expression of three protein in fatal cases was higher than that in survival cases (P<0.01). Conclusion The expression of EFGR, CD44v6 and PGP in colorectal carcinoma may be useful in prognosis of patients.
