基于TACE综合介入治疗中晚期原发性肝癌的效果及对患者生活质量的影响

目的探讨基于肝动脉化疗栓塞(TACE)综合介入对中晚期原发性肝癌(PLC)的治疗效果。方法选取我院收治的96例中晚期PLC患者为研究对象,根据手术方式的不同将其分为对照组(48例,TACE治疗)与观察组(48例,TACE+RFA+PEI+碘125粒子植入术治疗)。对比两组患者的手术效果。结果术后12、18、24个月,观察组患者的生存率明显高于对照组(P<0.05);术后2年,观察组患者的治疗总有效率为85.00%,明显高于对照组的63.33%(P<0.05);术后1周,两组患者的ALT、AST、GGT及TBIL水平均高于术前(P<0.05);但组间差异不显著(P>0.05);术后1年,两组患者的生理状况、社会/家庭状况、功能状况及情感状况评分及总分均高于术前,且观察组明显高于对照组(P<0.05)。结论基于TACE综合介入治疗中晚期PLC的效果显著,值得推广与应用。

Effect of comprehensive intervention based on TACE in primary liver cancer in middle and late stage and the influence on quality of life of patients

Objective To investigate the effect of comprehensive intervention based on transcatheter arterial chemoembolization(TACE)in primary liver cancer(PLC)in middle and late stage.Methods A total of 96 patients with PLC in middle and late stage treated in our hospital were selected as the study subjects,and divided into control group(48 cases,TACE treatment)and observation group(48 cases,TACE+RFA+PEI+ iodine 125 particle implantation treatment)according to the different surgical methods.The surgery effects between the two groups were compared.Results After 12,18,24 months after surgery,the survival rates of patients in the observation group were significantly higher than those in the control group(P<0.05);at 2 years after surgery,the total effective rate of treatment in the observation group was 85.00%,which was significantly higher than 63.33% in the control group(P<0.05).At 1 week after surgery,the levels of ALT,AST,GGT and TBIL in the two groups were higher than those before surgery(P<0.05);but the differences were not significant between the two groups(P>0.05).At 1 year after surgery,the physiological status,social/family status,functional status and emotional status scores and total scores of the two groups were higher than those before surgery,and those in the observation group were significantly higher than the control group(P<0.05).Conclusion Comprehensive intervention based on TACE in primary liver cancer in middle and late stage has remarkable effect,which is worthy of promotion and application.
