经胸超声心动图在急性胸痛鉴别诊断中的临床价值

目的探讨经胸超声心动图在急性胸痛鉴别诊断中的临床价值。方法选择2015年6月至2018年6月我院收治的100例急性胸痛患者为研究对象,均于超声检查室行经胸超声心动图检查或床旁超声心动图检查。之后患者均按胸痛中心制定的胸痛诊治流程进行诊治,最后比较经胸超声心动图诊断结果与出院诊断结果。结果 100例急性胸痛患者中,经胸超声心动图检查共发现异常89例,漏诊11例。发现的89例异常患者中,误诊8例。经胸超声心动图的诊断阳性率为89.0%,准确率为81.0%,误诊率为8.0%,漏诊率为11.0%。结论经胸超声心动图在鉴别诊断急性胸痛病因方面具有一定的价值,并可作为急性胸痛的一种初筛手段。

Clinical value of transthoracic echocardiography in differential diagnosis of acute chest pain

Objective To investigate the clinical value of transthoracic echocardiography in differential diagnosis of acute chest pain. Methods A total of 100 patients with acute chest pain admitted in our hospital from June 2015 to June2018 were selected as the study objects. All patients were examined by transthoracic echocardiography or bedside echocardiography in the ultrasound examination room. After that, the patients were diagnosed and treated according to the process of diagnosis and treatment of chest pain established by the Chest Pain Center. Finally, the diagnostic results of transthoracic echocardiography were compared with the results of discharge diagnosis. Results Among 100 patients with acute chest pain, 89 cases were found abnormal by transthoracic echocardiography, and 11 cases were missed. Of the 89 abnormal cases, 8 cases were misdiagnosed. The positive rate of diagnosis was 89.0%, accuracy rate was 81.0%,misdiagnosis rate was 8.0% and the missed diagnosis rate was 11.0% in the diagnosis of transthoracic echocardiography.Conclusion Transthoracic echocardiography has certain value in the differential diagnosis of acute chest pain, and can be used as a preliminary screening method for acute chest pain.
