MSCT后处理技术+MRI在胫骨平台骨折诊断中的应用价值

摘    要:目的研究探讨多层螺旋计算机断层扫描(MSCT)后处理技术+磁共振成像(MRI)在胫骨平台骨折诊断中的应用价值。方法选择2016年1月至2018年12月于本院就诊且怀疑为胫骨平台骨折的140例患者进行前瞻性研究,所有患者均行MSCT检查、MRI检查,采用后处理技术对CT图像进行重建,以手术诊断结果为参照,比较并分析MSCT、MRI、MSCT+MRI对胫骨平台骨折的诊断结果。结果以手术诊断结果为参照,MSCT+MRI对胫骨平台骨折的诊断灵敏度、特异度、准确度均高于MSCT、MRI,差异有统计学意义(P<0.05);MSCT与MRI对胫骨平台骨折的诊断灵敏度、特异度、准确度比较,差异无统计学意义(P>0.05)。一致性分析结果显示,MSCT+MRI对胫骨平台骨折的诊断结果与手术诊断结果之间的一致性良好,而MSCT、MRI诊断结果与手术诊断结果之间的一致性均为中等。MSCT+MRI对Ⅰ、Ⅱ、Ⅲ型胫骨平台骨折的诊断符合率均高于MSCT、MRI,差异有统计学意义(P<0.05);MSCT、MRI对Ⅰ、Ⅱ、Ⅲ型胫骨平台骨折的诊断符合率比较,差异无统计学意义(P>0.05);MSCT、MRI、MSCT+MRI对Ⅳ、Ⅴ、Ⅵ型胫骨平台骨折的诊断符合率比较,差异无统计学意义(P>0.05)。结论 MSCT后处理技术+MRI用于胫骨平台骨折诊断中具有良好的价值,不仅可灵敏检出胫骨平台骨折,还可对不同类型胫骨平台骨折予以鉴别区分。

Abstract:Objective To study the application value of multi-slice computed tomography(MSCT) post-processing technology+magnetic resonance imaging(MRI) in the diagnosis of tibial plateau fracture. Methods A total of 140 patients admitted in our hospital from January 2016 to December 2018 and suspected to be tibial plateau fracture were selected for prospective study. All patients were examined by MSCT and MRI, and CT images were reconstructed by post-processing technology. Referring to the results of surgical diagnosis, the diagnostic results of MSCT, MRI and MSCT+MRI for tibial plateau fracture were compared and analysed. Results Taking surgical diagnosis results as reference, the sensitivity,specificity and accuracy of MSCT+MRI in the diagnosis of tibial plateau fracture were higher than those of MSCT and MRI,the differences were statistically significant(P <0.05);there were no statistically significant differences in the diagnostic sensitivity, specificity and accuracy between MSCT and MRI for tibial plateau fracture(P>0.05). The results of consistency analysis showed that the diagnostic results of MSCT +MRI for tibial plateau fracture was in good agreement with that of surgical diagnosis results, while the consistency between MSCT and MRI diagnosis and surgical diagnosis results were moderate. The diagnostic coincidence rates of MSCT+MRI for type Ⅰ,Ⅱ and Ⅲ tibial plateau fractures were higher than those of MSCT and MRI, and the differences were statistically significant(P <0.05);there were no significant differences between MSCT and MRI in diagnosing type Ⅰ,Ⅱ and Ⅲ tibial plateau fractures(P >0.05);there were no statisticant differences in the diagnostic coincidence rates of MSCT, MRI, MSCT+MRI for type Ⅳ,Ⅴ and Ⅵ tibial plateau fracture(P>0.05).Conclusion MSCT post-processing technology+MRI has good value in the diagnosis of tibial plateau fracture. It can not only detect tibial plateau fracture sensitively, but also differentiate different types of tibial plateau fracture.
