肠内生态免疫营养联合生大黄在急性重症胰腺炎治疗中的应用

目的探讨早期经鼻空肠营养管鼻饲肠内生态免疫营养与生大黄对重症急性胰腺炎(Severe Acute Pancreatitis,SAP)的治疗价值。方法对2010年6月至2013年12月宣城市人民医院收治的86例SAP患者进行回顾性分析,按治疗方法不同分为研究组(46例)和对照组(40例),两组患者人院后均给予常规治疗,研究组在患者入院2天内放置液囊空肠营养管至空肠,早期鼻饲生大黄与肠内生态免疫营养剂。观察患者的营养状况,免疫功能,并发症情况等。结果两组在治疗后血清白蛋白,总淋巴细胞计数,血红蛋白差异有统计学意义,(P0.05)。研究组肠道功能恢复较早,且并发症、感染率、住院时间和费用低于常规治疗组(均P0.05)。结论对SAP患者早期予以鼻饲生大黄与肠内生态免疫营养剂,是安全实用有效的,能有效促进SAP患者肠道功能恢复,改善患者营养状况及免疫功能,缩短住院时间,值得临床应用。

The application of enteral ecoimmunonutrition and Rude Rhubarb in treating Severe Acute Pancreatitis

Objective To discuss the significance of supplementing enteral ecoimmunonutrition and Rude Rhubarb treating Severe Acute Pancreatitis（ SAP） by nasojejunal feeding. Methods 86 patients of SAP who were hospialized in the People＇s hospital of xuancheng during June 2010 and December 2013 were retrospectived analysis. All patients were divided into study group and control group according to the different therapy. patients of two groups were all given conventional treatment,patients in study group were given early enteral immunizational nutrition and Rude Rhubarb through nose-jejunum nurient canal in patients hospitalized 2d. The nutrition and immune index and the rates of complications were analyzed. Results The albumin,total number of lymphocyte,hematoglobin of two groups was significantly differen after the therapy,（ P〈0. 05） Recovery time of Intestinal function was earlier in study group. The incidence rate of complications,infection rate,length of hospital stay and hospital cost in study group were lower than control group.Conclusion It is safe,efficient and feasible of the early entera ecoimmunonutrition in severe acute pancreatiris（ SAP） through nose-jejunum nurient canal. Early enteral ecoimmunonutrition Can effectively promote the recovery of intestinal function,improve the patient nutrition,immune function,shorter hospital stay,it is worthy of application.
