数字化乳腺摄影和超声对小乳癌的诊断价值

目的：探讨数字化乳腺摄影与超声对小乳癌的诊断价值。方法：60例≤2．0cm（包括未形成明确肿块的早期乳腺癌）乳腺结节的乳腺摄影和超声资料分别由影像医师和超声医师采用双盲法进行分析，定性诊断，并与病理结果对照。结果：60例患者，病理证实乳腺癌36例，良性病变24例；乳腺摄影和毡声诊断小乳腺癌的特异性、准确性、敏感性分别为95％、90％、83．3％和83．3％、70％、61．1％；二者诊断小乳癌的敏感性、准确性差异有显著性（P〈0．05）；对直径≤1．0cm的病灶，数字化摄影的准确性明显高于超声（P〈0．05）。结论：在对小乳癌的定性诊断方面，数字化乳腺摄影应作为首选，但二者结合有利于发现更早期的乳腺癌。

Objective ：To discuss the diagnostic value of Digital Mammography（ DM ） and Color Doppler Ultrasonography （CDUS） for small breast carcinoma. Methods The Digital Mammography findings and ultrasonographic images of 60 cases with breast nodules less than 2. Ocm in diameter were analyzed and diagnosed by experienced radiologists and ultrasonography doctors with double blind method and compared with pathological results. ResultsAmong 60 cases,The specificity, accuracy and sensitivity of DM in diagnosis of small breast cancer were 95%, 90% and 83.3%, but in CDUS 83.3%, 70% and 61.1% respectively. The sensitivity and accuracy of DM in diagnosis of small breast cancer were significantly higher than that of CDUS, especially for the lesions 〈 1cm in size （p 〈 0.05）. Conclusion Digital mammography can be used as the first diagnostic method for small breast cancer , but combining DM and CDUS can heighten the assessment of early breast cancer.
