| 摘 要： | 目的探讨胆管镜在腹腔镜胆囊切除术(LC)中胆管疾病诊治的临床价值。方法在腹腔镜胆囊切除术(LC)术中,对127例有胆总管探查指征者同时行胆管镜探查,根据探查结果而选择不同的治疗方式。结果患者均在腹腔镜胆囊切除术中应用胆管镜探查,胆管系统有阳性发现者34例,占26.8%。其中,胆管结石24例,术中取净21例;肝内胆管狭窄4例和Oddi括约肌狭窄3例,均进行球囊导管扩张治疗,4例术中成功扩张。活检证实,壶腹部癌2例和肝门部胆管癌1例,均中转开腹。结论胆管镜可直视胆管内部,在腹腔镜胆囊切除(LC)中应用,既可降低胆管疾病漏诊率,也可同时胆管取石和治疗胆管狭窄,并为选择合理的手术方式提供参考。

| Abstract: | Objective To discuss the clinical value of choledochoscopy for bile duct exploration during laparoscopy in diagnosis and treatment. Methods 127 patients who had indication of exploration of choledochoscopy were received the laparoscopy choledochoscopy according to varied results the treatment method was selected. Results The positive finding was 34 in 127 at rate of 26.8%. Among them 21 cases of bile stone were taken out. There were 4 cases bile duct stenosis, 3 cases Oddi's sphincter stenosis with balloon dilation. The 3 cases were turned to abdominal operation in 2 ampulla and bile duct carcinoma. Conclusion The choledochoscopy could view inside of bile duct. The LC application could decrease misdiagnosis rate and take out stone and dilate the stenosis. It might be reasonable reference for operation. |
