独活寄生汤提取物治疗椎间盘退变性疾病机理实验研究

摘    要:目的:探讨独活寄生汤治疗腰椎间盘退变性疾病的机理。方法:选取6-8月龄雄性新西兰白兔24只,随机分为3组各8只。退变组和中药组白兔制作椎间盘退变模型,中药组给予独活寄生汤的水煎剂喂养,退变组、对照组不予中药喂养,8周后,每组取1只白兔对其进行椎间盘切片观察;24周后,对每组剩余的白兔取L4-L5椎间盘染色观察,培养软骨终板细胞,进行细胞形态学的观察,并测其上清液中的白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)。结果:对照组软骨终板矢面观,周边较厚而中央较薄,中央部血管芽较多且较膨大,周边部血管较少且较细小。对照组软骨终板结构,可见深层软骨细胞胞体较大,胞核相对较小,呈圆形或椭圆形而表层软骨终板细胞胞体扁平,胞核亦呈扁平状,在软骨终板与椎体股之间可见潮标。中药组软骨终板厚,细胞数目较多,血管芽直接与软骨终板接触,潮标未见前移。退变组软骨终板较对照组明显变薄,细胞数亦减少,潮标明显前移,血管芽周围钙化,包裹血管芽大部分。退变组软骨终板变薄,细胞数减少,染色较淡。中药组软骨终板细胞培养液中IL-6和TNF-α的含量均高于对照组和退变组(P〈0.05),IL-1β的含量高于对照组(P〈0.05),但低于退变组(P〈0.05)。结论:独活寄生汤提取物对腰椎间盘退变模型白兔腰椎间盘IL-1β、IL-6、TNF-α有促进作用,可能为其治疗腰椎间盘退变性疾病的机制之一。

Therapeutic Mechanism of Duhuo Jisheng Decoction for Lumbar Degenerative Disc Disorder in Rabbits

Abstract:Objective:To investigate the therapeutic mechanism of Duhuo Jisheng Decoction for lumbar degenerative disc disorder.Methods:Twenty-four male New Zealand white rabbits aged 6-8 months were equally randomized into normal control group,model group,and Chinese medicine group.Rabbits except the normal control group were induced lumbar degenerative disc disorder,and Chinese medicine group was given the decoction of Duhuo Jisheng Decoction orally.After treatment for 8 weeks,one rabbit from each group was executed for the preparation of intervertebral disc slice.After treatment for 24 weeks,all of the left rabbits in the three groups were executed for L4-L5 disc staining and morphological observation,and then the carti-lage lamina terminalis cells were cultured.The morphological features of the cultured cells were observed,and the levels of interleukin-1β(IL-1β),IL-6 and tumor necrosis factor-α(TNF-α)in the supernatant solution were observed.Results:The sagittal plane view of the cartilage lamina terminalis of normal control group was shown as thick surrounding areas with less small vascular buds and thin central part with more swollen vascular buds.The structure of cartilage lamina terminalis of the control control group presented as large round or oval-shaped cartilage cell body with a relatively small nucleus,flat nucleus and flat surface of the cartilage lamina terminalis cell body,and a tidemark between the cartilage lamina terminalis and vertebral body.In Chinese medicine group,the cartilage lamina terminalis was thick with more cells,vascular buds was contacted directly with the cartilage lamina terminalis,and tidemark was not found.In the model group,the cartilage lamina terminalis became thinner,the number of cells decreased and the cells were stained lighter,the tidemark was more advanced than in the normal control group,and the areas around vascular buds around were calcificated,surrounding the majority of vascular buds.IL-6 and TNF-αlevels of Chinese medicine group were higher than the model group and normal control group(P<0.05),IL-1β level was between the control group and the degenerative group(P<0.05).Conclusion:Duhuo Jisheng Decoction has an effect on promoting the excretion of IL-1β,IL-6 and TNF-αin lumbar intervertebral disc of rabbits with lumbar degenerative disc disorder,which may be one of its therapeutic mechanisms.
