TCD与SPECT对MID、DAT脑血流的初步观察

用经颅多普勒探测３３例多梗塞痴呆，２１例Ａｌｚｈｅｉｍｅｒ型痴呆和２４例同龄健康人大脑中动脉，大脑前动脉，大脑后动脉平均流速，并用单光子发射计算机断层显像相对定量法测定局部脑血流量，结果发现３３例ＭＩＤ病人Ｖｍ减慢，ＤＡＴ病人９例减慢；ＭＩＤ病人ＭＣＡ的Ｖｍ减慢比ＤＡＴ病人更明显，全部ＭＩＤ，ＤＡＴ病人均见ｒＣＢＦ降低。提示ＭＩＤ病人ｒＣＢＦ降低主要由于脑血管病变所致，ＤＡＴ病人ｒＣＢＦ降低主要因

Transcranial Doppler Sonography and SPECT in the assay of brain blood flow in multi-infarct dementia and Alzheimer's dementia

Transcranial Doppler Sonography (TCD) was used to assay the Vm in MCA, CA and PCA in 33 patients with multi-infarct dementia (MID) and 21 with Alzheimer's dementia (DAT) and 24 healthy individuals, and the relative quantitative method by SPECT was used to assay rCBF as well. It showed that Vm in 33 patients with MID and in 9 with DAT slowed down; furthermore, Vm of MCA in MID slowed down more significantly than that in DAT. rCBF decreased in all patients with MID and DAT. It suggested that the decrease of rCBF in MID is mainly due to the cerebrovascular lesion and in DAT due to the less needed blood flow induced by the descent of cerebral cortex metabolism. TCD and SPECT are valuable in the diagnosis of MID and DAT, and SPECT may be more sensitive than TCD in the diagnosis of rCBF in DAT.
