老年腰椎间盘突出症患者介入治疗的随访研究

目的:回顾性分析介入治疗老年腰椎间盘突出症的疗效.方法:87例老年腰椎间盘突出症患者分为3组,均采用不同介入方法治疗,并进行6个月的随访.结果:总有效率95.4%,其中经皮腰椎间盘切除术(PLD组)有效率93.9%;化学溶核术(CNL组)96.3%;二者结合的双介入术(DIT组)96.4%,3组均无严重并发症发生.结论:介入治疗老年腰椎间盘突出症安全有效.双介入疗法的显效率优于单种治疗方法,对于双节段或单节段突出程度较重的老年患者宜采用双介入疗法.

The Follow-up Study of Interventional Therapy in Aged Patients with Lumbar Intervertebral Disc Herniation

Objective: To investigate the effect of the interventional therapy in aged patients with lumbar intervertebral herniation (LIDH). Methods: Eighty-five aged patients with lumbar intervertebral disc herniation were treated by three interventional methods according to their different conditions: 33 cases were subjected to percutaneous lumbar disectomy (PLD), 26 cases to chemonucleolysis (CNL), and the remaining 28 cases to combined therapy (PLD+CNL). Results: After follow-up for 6 months, the effective rate in PLD group, CNL group and PLD+CNL group was 93.9 %, 96.3 % and 96.4 % respectively. The total effective rate was 95.40 % (83 cases). There were no severe complications. Conclusion: Interventional therapy is a safe and effective management for aged patients with lumbar intervertebral disc heniation. Combined therapy is superior to PLD or CNL alone. It is suggested the combined therapy could be used for double segment herniation or severe herniation in aged patients
