重组牛碱性成纤维细胞生长因子联合玻璃酸钠对糖尿病性白内障超声乳化术后干眼症的影响分析

探讨重组牛碱性成纤维细胞生长因子联合玻璃酸钠治疗对糖尿病性白内障患者超声乳化术后干眼症的影响。方法　选择２０１０年１月—２０１３年１０月就诊的糖尿病性白内障行超声乳化术患者１０６例（１４５只眼），随机数字表法分为对照组（Ａ组５０只眼）、玻璃酸钠治疗组（Ｂ组４８只眼）和联合治疗组（Ｃ组４７只眼）。在术前、术后１ｄ、术后１周、术后１个月进行主观干眼症状、泪膜破裂时间（ＢＵＴ）、基础泪液分泌试验（ＳＩｔ）及角膜荧光素染色（ＦＬ）检查。结果　术后１月，Ａ组干眼主观症状、ＢＵＴ和ＦＬ与术前相比差异仍有统计学意义（Ｐ＜００５）。Ｂ组术后１周ＳＩｔ、ＦＬ和术后１个月干眼主观症状、ＢＵＴ接近术前水平（Ｐ＞００５）。Ｃ组术后ｌ周干眼主观症状、ＢＵＴ、ＳＩｔ和ＦＬ均接近术前水平（Ｐ＞００５）。结论　糖尿病性白内障患者行超声乳化手术后早期使用重组牛碱性成纤维细胞生长因子联合玻璃酸钠治疗，更有利于改善症状、恢复泪膜稳定性。

Clinical study on the effect of combination therapy of sodium hyaluronate and recombinant bovine basic fibroblast growth factor after phacoemulsification in diabetics

To study the efficacy of the corubined treatment with sodium hyaluronate ( HA) and recombinantbovine basic fibroblast growth factor( r - bFGF) for II - diabetic patients after phacoemulsification. Methods A total of106 11 - diabetic patients undergoing phacoemulsification were randomized divided into control group ( group A,50 eyeswith conventional therapy) , single treatment Group ( group B ,48 eyes with conventional therapy and HA) and combinedtreatment group ( group C ,47 eyes with conventional therapy , HA and r - bFGF ) . Subjective symptoms , tear film break -up time( BUT) .Schirmer I test ( SIt) and corneal fluorescein staining ( FL) were measured preoperatively and I day , Iweek and I month postoperatively. Results In all groups. the BUT reduced greatly , SIt value and FL increased significantlyafter phacoemulsification. Compared with preoperations in group A, subjective complains of dry eye increased and theBUT reduced in I day, I week and I month postoperatively. So did FL. SIt value returned at postoperative value at Imonth after operations( P < 0. 05 ). In group B , SIt value and FL returned to postoperative value at postoperative I week.SO did subjective complains of clry eye and BUT at postoperative I month( P > 0. 05 ). There were no significant differ-ences in subjective complains of dry eye,BUT. SIt and FL in group C in I week after phacoemulsification ( P > 0. 05 ). Conclusion It can be more effective to relieve the dry eye symptoms for II diabetic patients to use combined treatmentwith HA and r - bFGF at early stage after phacoemulsification.
