既往盆腹腔外科治疗史对妇科 患者微创手术效果的影响

探讨既往盆腹腔外科治疗史对妇科患者微创手术效果的影响。方法　选择我院２０１２年１月至 ２０１５年１月收治的因妇科疾病具有盆腹腔镜手术史后又行微创手术的患者８０例为观察组，选择同期无盆腹腔 手术史患者８０为对照组。比较２组患者盆腔粘连情况以及手术治疗情况。结果　观察组患者有５３例发盆腹 腔粘连，发生率为６６３％，显著高于对照组的１２５％，具有统计学意义（Ｐ＜００５）；研究组盆腹腔粘连轻度２７ 例，占５０９％、中度粘连１６例，占３０２％、重度粘连１０例，占１８９％，对照组粘连轻度占６００％、中度粘连占 ３００％、重度粘连占１００％，２组比较差异均无统计学意义（Ｐ＞００５）。本研究结果显示观察组患者手术时间、 术中出血量、住院时间虽然稍高于对照组患者，但２组患者比较均有显著差异（Ｐ＞００５），而在中转开腹、手术 成功率方面均无统计学意义（Ｐ＞００５）。结论　既往有盆腹腔手术史的患者中转开腹率增加，盆腔粘连增加手 术难度，但通过细致操作，再次手术同样具有良好的可行性和安全性。

to explore the history of basin of abdominal surgical treatment of gynecological patients , and the effects of minimally invasive surgery. Methods select our hospital between January 2012 and January 2015 because of disease of department of gynaecology line basin after laparoscopic surgery and minimally invasive surgery patients to ob- serve 80 cases of group, choose the same period there is no history of basin of laparoscopic surgery for 80 patients with the control group. Comparing the 2 groups of patients with pelvic adhesions and surgical treatment. And the observation group of 53 patients with abdominal cavity adhesion, with statistical significance ( P < 0. 05 ) ; Team basin no statistically significant difference were observed in the two groups ( P > 0. 05 ). This study results show that the observation group of patients with operation time , intraoperative blood loss, hospital stay , although slightly higher than the control group pa- tients , but the patients were compared with two group were significant difference ( P > 0. 05 ) , surgical success rate had no statistical significance ( P > 0. 05 ). Conclusion Always has a history of basin of abdominal surgery patients transfer laparotomy rate increase, pelvic adhesion increase operation difficulty, but through careful operation, again also has a good feasibility and safety of operation.
