自拟益气化瘀通络方对原发性肾病综合征激素维持治疗阶段患者凝血功能及血脂的影响

目的观察自拟益气化瘀通络方对原发性肾病综合征激素维持治疗阶段患者凝血功能及血脂的影响。方法将50例已确诊的原发性肾病综合征在激素维持治疗阶段的患者随机分为2组,治疗组25例给予益气化瘀通络中药联合激素治疗,对照组25例给予单纯激素治疗,2组连续治疗1个月,观察血凝、血脂指标和血尿蛋白指标。结果治疗组治疗前后比较在降低血浆纤维蛋白原(Fib)、升高抗凝血酶III(AT-III),降低血清总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)和减少24 h尿蛋白(24 h-UP)、升高血白蛋白(Alb)方面有显著性差异(P<0.01);而对照组治疗后各项指标均无显著改善。治疗组治疗后与对照组治疗后比较在降低血浆Fib、升高AT-Ⅲ,降低血清TC、TG和减少24 h-UP方面疗效更为显著(P<0.05或P<0.01)。结论自拟益气化瘀通络方可以明显改善原发性肾病综合征激素维持治疗阶段患者的血液高凝状态和脂代谢紊乱。

Effects of Yiqihuayutongluo decoction on blood coagulation and blood lipid in patients with primary nephrotic syndrome in hormone maintenance stage

Objective To observe the effect of Yiqihuayutongluo decoction on primary nephrotic syndrome hormone maintain influence treatment and coagulation function in patients with blood lipid phase.Methods Fifty patients who had been diagnosed with primary nephrotic syndrome and received maintained treatment of hormone were randomly divided into two groups,25 cases in the treatment group were given Yiqihuayutongluo decoction combined hormone therapy,25 cases in the control group were treated with hormone therapy,2 groups received 1 months of continuous treatment,and then observe blood coagulation,blood lipid and blood and urine protein index.Results In treatment group,compared before and after treatment,it has significant difference in reducing plasma fibrinogen（Fib）,increase of antithrombin III（AT-III）,lower serum total cholesterol（TC）,triglyceride（three TG）,low density lipoprotein cholesterol（LDL-C） and reduce 24 h urine protein（24h UP）,elevated serum albumin（Alb）（P 〈0.01）;while the control group after treatment,all indexes were not significant improvement.Treatment group and control group after treatment,treatment group has more significant effect in reducing plasma Fib,increased AT-III,reduce serum TC,TG and reduce 24 h UP（P〈 0.05 or P 〈0.01）.Conclusion It proved that the Yiqihuayutongluo decoction could obviously improve the blood hypercoagulability and disorders of lipid metabolism in patients with the primary nephrotic syndrome in treatment of hormone maintain stage.
