国产氯吡格雷治疗冠心病疗效及安全性的Meta分析

目的系统评价国产氯吡格雷(泰嘉)在冠心病治疗过程中的疗效及安全性。方法计算机检索中国生物医学文献数据库(CBMdisc)、中国期刊全文数据库(CJFD)、中文科技期刊全文数据库(VIP)、万方数据库,检索日期为2000年1月一2014年1月,同时阅读各纳入研究的参考文献.纳入国产氯吡格雷(泰嘉)和进口氯吡格雷(波立维)治疗冠心病的随机对照试验(RCT)。评价纳入研究的方法学质量并进行资料提取后,采用RevMan5.1软件进行Meta分析。结果共纳入24个RCT,包括7 577例患者。Meta分析结果显示,泰嘉和波立维血小板聚集率(WMD=-0.82,95%CI-3.00¹.36,P=0.46)、主要心血管不良事件(RR=0.91,95%CI 0.771.36,P=0.46)、主要心血管不良事件(RR=0.91,95%CI 0.77¹.07,P=0.24)及出血发生率(RR=0.96,95%CI 0.781.07,P=0.24)及出血发生率(RR=0.96,95%CI 0.78¹.20,P=0.74)、过敏发生率(RR=1.09.95%CI 0.721.20,P=0.74)、过敏发生率(RR=1.09.95%CI 0.72¹.66,P=0.69),差异均无统计学意义。结论国产氯吡格雷治疗冠心病疗效确切,使用安全。

Meta-analysis of the efficacy and safety in treatment of coronary heart disease with domestic made clopidogrel

Objective To system review domestic clopidogrel（Taijia） efficacy and safety in the treatment of coronary heart disease.Methods Computer retrieval Chinese biomedical literature database（CBMdisc）,China journal full text database（CJFD）,Chinese Technology Journal Full-text Database（VTP）,Wanfang database,retrieval date from 2000 January to 2014 January,at the same time reading the references of eligible studies,including the domestic clopidogrel（Taijia）and imported clopidogrel（Boliwei） randomized controlled trial in the treatment of coronary heart disease（RCT）.Study quality was evaluated and then extracted data,Meta-analysis was performed with RevMan5.1 software.Results A total of 24 RCT,including 7 577 cases were enrolled.Meta-analysis showed that the platelet aggregation rate（WMD =-0.82,95%CI-3.00-1.36,P =0.46）,major adverse cardiovascular events（RR =0.91,95%CI0.77- 1.07,P =0.24） and hemorrhage（RR=0.96,95%CI 0.78-1.20,P =0.74）,incidence of allergies（RR = 1.09,95%CI 0.72- 1.66,P =0.69）were revealed no significant differences between the two medicine.Conclusion Domestic made clopidogrel has reliable efficacy and is safe in the treatment of CHD.
