后腹腔镜技术治疗输尿管结石

目的探讨后腹腔镜输尿管切开取石术治疗输尿管结石的临床价值.方法2002年4月到2004年5月对5例单侧输尿管中、上段结石患者（其中1例行ESWL术失败）行后腹腔镜输尿管切开取石术.结果5例患者取石均获成功,手术时间60～140 min,平均120 min,术中出血量20～40ml,平均25ml.术后漏尿3～5 d;术后5 d拔除后腹腔引流管,术后住院7～8 d.随访4～12个月,患者输尿管积水明显好转,无输尿管狭窄.结论后腹腔镜输尿管切开取石术适合既往需要开放性输尿管切开取石术的病例,效果确切,损伤少,患者术后恢复快,可部分取代传统的开放性手术.

Retroperitoneal laparoscopy to treat ureteral calculi: A Report of 5 cases

Objective To evaluate the clinical effects of retroperitoneal laparoscopic ureterolithotomy.Methods From April 2002 to May 2004,5 patients were treated with retroperitoneal laparoscopic ureterolithotomy.All the patients suffered from calculi in superior or middle segmental ureter.One of the patients was treated with ESWL before.Results All the 5 cases underwent retroperitoneoscopic ureterolithotomy successfully.The operative time ranged from 60 to 140 min(mean 120 min);the intraoperative blood loss ranged from 20 to 40 ml(mean 25 ml).All the 5 cases experienced 3 to 5 days' leakage of urine.The retroperitoneal drainage-tubes were removed 5 days after operation.The postoperative hospital stay was 7 to 8 days.During the follow-up of 4-12 months,hydronephrosis and hydroureterosis were lessened and no ureteric stricture was found.Conclusions Retroperitoneal laparoscopic ureterolithotomy is suitable for ureteral calculi which treated by open surgery before.It is effective,safe,minimal invasive,rapid postoperative rehabilitation and it can partially replace conventional open surgery in part.
