结肠癌中DLL4/Notch-1的表达及意义

目的：探讨 DLL4、Notch1在结肠癌中的表达及其临床意义。方法：收集十堰市人民医院病理科2009年1月至2012年10月存档的结肠蜡块标本95例，其中正常黏膜组织8例，结肠癌70例，包括高分化腺癌30例、中分化腺癌25例、低分化腺癌15例；结肠腺瘤17例。运用免疫组织化学 SP 法检测DLL4、Notch1在组织中的表达，分析其表达水平与临床病理之间的关系。结果：（1）在结肠正常黏膜-腺瘤-癌序列中，DLL4的阳性率呈递增趋势，其在结肠癌组中的表达率高于正常黏膜组（P〈0.05）；（2） Notch-1的阳性率在结肠正常黏膜-腺瘤-癌序列中亦呈递增趋势，其在结肠癌组中的表达率高于正常黏膜组（P〈0.05）；（3）DLL4、Notch-1在结肠癌不同分化程度中的表达率不同，分化越低，阳性率越高（P〈0.01）。结论：DLL4/ Notch1的表达与结肠癌的发生、发展有关，可为结肠癌基因治疗提供靶点。

The expression and clinical significance of Notch-1 and DLL4 in colonic-mucosa in patients with colon cancer

Objective： The present experiment is to investigate the expression of Notch-1 and DLL4 in colonic mucosa in patients with colon cancer for the clinical diagnosis and useful prognostic indicator. Methods The 95 cases of paraffin block from the department of pathology of the Shiyan people hospital from January 2009 to October 2012, 17colon adenoma, 70 colon cancer with 30 well-differentiated, 25 moderately differentiated and 15 poorly differentiated, 8 cases of normal colon, Notch-1 and DLL4 expression was assessed by immunohistochemistry method. Result （1）we demonstrated that the positive rate of Notch-1 in the sequence of normal colon mucosa-adenoma-carcinoma was gradually increased. The positive rate of Notch11 in colon cancer was higher than that of normal muclsa group（P〈0.05）.the positive rate of DLL4 in the sequence of normal colon mucosa-adenoma-carcinoma was gradually increased. The positive rate of DLL4 in colon cancer group was higher than that of colon adenoma group and normal colon mucosa group（（P〈0.05）.（2）we found that the positive rate of Notch-1 in colon cancer concerned with tumor size, different depth of invasion, lymph node metastasis and extent of defferentiation（P〈0.05）.The positive rate of DLL4 in colon cancer concerned with the differentiation degree and lymph node metastasis （ P 〈 0. 05）. The expression of Notch-1 and DLL4 by rank sum test was distinguished at distinct differentiation degree in colon cancer. The lower differentiation, the higher positive rate was（P〈0.05）.（3）we also found that the expression of Notch-1 and DLL4 in colon cancer had positive correlation （P 〈 0. 01）. Conclusion The high expression of Notch-1 and DLL4 in colon mucosa is related with the pathogenesis and development of colon cancer, which maybe become a potent target for colon mucosa treatment.
