98例老年癌性胸液与结核性胸液的纤维支气管镜检查分析

目的 :观察纤维支气管镜检查对老年癌性胸腔积液的诊断意义并探讨其对老年癌性胸液与老年结核性胸液的鉴别诊断价值。方法 :对近 5年收治的 98例 6 0岁以上癌性胸液和结核性胸液患者均予PENTAX 90型纤支镜检查并行活检和刷检 ,对退镜后咯出的分泌物涂片送检并找结核杆菌。结果 :除支气管腔内出血在两组胸腔积液中无显著性差异外 (P >0 .0 5 ) ,其它各项纤支镜下表现均有显著性差异 (P <0 .0 5或P <0 .0 1)。纤支镜检查对老年癌性胸液和老年结核性胸液的诊断率分别为 74.1%和 42 .5 %。结论 :纤支镜检查是确诊老年癌性胸液的重要手段 ,对老年癌性胸液与结核性胸液的鉴别诊断具有重要的价值。

Fiberoptic bronchoscopy of 98 cases of the aged carcinomatous hydrothorax and tuberculous pleural effusion

Purpose:To study the value of fiberoptic bronchoscopy in the accurate diagnosis of the aged carcinomatous hydrothorax and the differential diagnosis between the aged carcinomatous hydrothorax and the aged tuberculous pleural effusion.Method:98 cases of the aged patients with carcinomatous hydrothouax and tuberculous pleural effusion were examined by PENTAX-90 fiberoptic bronchoscope and were biopsied and brush-biopsied.After withdrawing the fiberoptic bronchoscope the expectoratic secretion were examined for mycobacterium.Result:With the only exception that the bleeding inside the bronchial tube showed no obvious difference (P>0.05),all checking items presented marked difference (P<0.05 or P<0.01).The diagnostic accordance of the aged carcinomatous hydrothorax and tuberculous pleural effusion amounted to 74.1% and 42.5% respectively.Conclusion:The fiberoptic bronchoscopy was a valuable procedure in the diagnosis of the aged carcinomatous hydrothorax and was importance to the differential diagnosis between the aged carcinomatous hydrothorax and tuberculous pleural effusion.
