CK19、CK20 mRNA在上皮性恶性肿瘤患者外周血中的表达及其意义

摘    要:目的:分析上皮性恶性肿瘤患者外周血中CK19、CK20mRNA的表达水平,探讨其临床病理意义。方法:随机选择和收集53例上皮性恶性肿瘤患者的外周血,应用巢式RT-PCR法联合检测其CK19、CK20mRNA表达情况,另取15例上皮性肿瘤组织的为阳性对照,15例非肿瘤患者外周血为阴性对照。结果:53例肿瘤患者外周血中CK19阳性率为35.8%(19/53,CK20阳性率41.5%(22/53),两者至少1项阳性者69.8%(31/53)。15例肿瘤组织中CK20阳性率为100%(15/15),CK19阳性率86%(13/15);15例阴性对照CK19阳性2例,CK20阳性1例,结合肿瘤的临床病理资料分析,肿瘤患者外周血CK19、CK20mRNA阳性表达与肿瘤浸润深度,临床分期及淋巴结转移存在明显的相关性,而与肿瘤患者的年龄,性别,肿瘤分化程度无明显相关。结论:CK19、CK20mRNA可作为检测上皮性肿瘤外周血微转移的靶基因。联合检测肿瘤患者外周血CK19、CK20mRNA可增加外周血循环癌细胞的检测率。

Abstract:Objective To study the abnormal expression of CK19,CK20 mRNA in the peripheral blood for epithelial malignancies patients,and to explore its clinical significance.Methods A total of 53 preoperative peripheral blood specimens were obtained from patients with epithelial malignancies.RT?PCR assay was applied to detect the expression of CK19,CK20 mRNA in the peripheral blood.As control,15 fresh tumor tissues and blood samples from 15 patients with inflammatory disease,were also obtained.Results The positivity for CK19 and CK20 mRNA expression were 35.8%(19/53) and 41.5%(22/53),respectively,69.8%(31/53) were positive of either one in peripheral bood from patients with epithelial cancers.In primary tumors,15 tumor tissues were all positive for CK20 mRNA,and 86%(13/15) were positive for CK19 mRNA.In blood samples from nonmalignant control subjects,2 were positive for CK19,and only one was positive for CK20.The expression of the two genes (CK19 and CK20) were significantly correlated with clinical pathologic stage,lymph node metastasis and depth of invasion,but had no coincidence with differentiation,sex and age of the patients.Conclusion These data indicate the CK19,CK20 mRNA expression may be potential biomarkers for detecting micrometastasis and recurrence in peripheral blood with epithelial cancers.
