泼尼松联合丙硫氧嘧啶治疗甲状腺功能亢进症的临床研究

目的探究醋酸泼尼松片联合丙硫氧嘧啶片治疗甲状腺功能亢进症的临床疗效。方法选取2015年6月—2017年7月天门市第一人民医院收治的146例甲状腺功能亢进患者为研究对象,所有患者随机分为对照组和治疗组,每组各73例。对照组口服丙硫氧嘧啶片,100 mg/次,3次/d。治疗组在对照组治疗的基础上口服醋酸泼尼松片,30 mg/d,3次/d。两组患者均连续治疗1个月。观察两组的临床疗效,比较两组的血清激素水平、抗体水平。结果治疗后,对照组和治疗组的总有效率分别为83.56%、98.63%,两组比较差异有统计学意义(P0.05)。治疗后,两组患者的促甲状腺激素(TSH)水平升高,游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)水平均降低,且治疗组患者的TSH水平明显比对照组高,FT3、FT4水平比对照组显著低,两组比较差异明显(P0.05)。治疗后,两组患者的甲状腺刺激性抗体(TSAb)水平显著降低,甲状腺刺激阻断性抗体(TSBAb)水平显著升高,同组治疗前后比较差异有统计学意义(P0.05);且治疗组患者TSAb水平降低幅度明显大于对照组,TSBAb水平升高幅度显著大于对照组,两组比较差异明显(P0.05)。结论醋酸泼尼松片联合丙硫氧嘧啶片治疗甲状腺功能亢进症具有较好临床疗效,可调节甲状腺激素水平,改善机体免疫功能,安全性较好,具有一定的临床推广应用价值。

Clinical study on prednisone combined with propythiouracil in treatment of hyperthyroidism

Objective To investigate the clinical efficacy of Prednisone Acetate Tablets combined with Propythiouracil Tablets in treatment of hyperthyroidism. Methods Patients (146 cases) with hyperthyroidism in Tianmen First People''s Hospital from June 2015 to July 2017 were randomly divided into control and treatment groups, and each group had 73 cases. Patients in the control group were po administered with Propythiouracil Tablets, 100 mg/time, three times daily. Patients in the treatment group were po administered with Prednisone Acetate Tablets on the basis of the control group, 30 mg/time, three times daily. Patients in two groups were treated for 1 month. After treatment, the clinical efficacies were evaluated, and serum hormone levels and serum antibody levels in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 83.56% and 98.63%, respectively, and there was difference between two groups (P<0.05). After treatment, the levels of TSH in two groups were significantly increased, but the levels of FT3 and FT4 in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the serum hormone levels in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of TSAb in two groups were significantly decreased, but the levels of TSBAb in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05). And the antibody levels in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). Conclusion Prednisone Acetate Tablets combined with Propythiouracil Tablets has clinical curative effect in treatment of hyperthyroidism, can adjust the level of thyroid hormone, and improve the immune function of the body, with good safety, which has a certain clinical application value.
