基于快速康复理念下腹腔镜胆囊切除手术患者的麻醉管理研究

目的:探讨快速康复外科理念在腹腔镜胆囊切除术麻醉管理中的应用效果。方法:将2019年1月—2019年12月收治的80例进行腹腔镜胆囊切除术的患者均分为观察组和对照组,每组40例。对照组患者采用传统麻醉管理,观察组患者采用基于快速康复理念下腹腔镜胆囊切除手术麻醉管理,观察对比两组患者的应激反应、术后麻醉苏醒时间、排气时间及并发症发生率。结果:麻醉前两组血糖和血清皮质醇比较差异无统计学意义(P>0.05)。在气腹成功时、术中以及手术结束时,观察组血糖和血清皮质醇均明显低于对照组,差异有统计学意义(P<0.05)。观察组麻醉苏醒时间、首次排气时间均明显短于对照组,差异有统计学意义(P<0.05)。观察组的并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论:加速康复外科理念应用于腹腔镜胆囊切除术麻醉管理,能够有效缓解围术期的应激反应,缩短术后麻醉苏醒时间和排气时间,降低并发症发生率,有助于患者快速恢复。

Study on anesthesia management of patients undergoing laparoscopic cholecystectomy under the concept of rapid rehabilitation

Objective:To explore the effect of anesthesia management in patients undergoing laparoscopic cholecystectomy under the concept of rapid rehabilitation.Methods:Eighty patients who underwent laparoscopic cholecystectomy in hospital from January 2019 to December 2019 were selected as the research object,and they were evenly divided into an experimental group and a control group.The patients in the control group were managed by traditional anesthesia.The patients in the experimental group were managed by anesthesia under laparoscopic cholecystectomy based on the concept of rapid rehabilitation.The stress response,postoperative anesthesia recovery time,exhaust time and the incidence of complications were observed and compared between the two groups.Results:There was no significant difference in blood glucose and serum cortisol content between the experimental group and the control group before anesthesia(P>0.05).During the success of pneumoperitoneum,intraoperatively and postoperatively,the blood glucose and serum cortisol contents of the experimental group were significantly lower than those of the control group,with statistical significance(P<0.05).The anesthesia recovery time and the first exhaust time of the experimental group were significantly shorter than that of the control group,with statistical significance(P<0.05).The incidence of complications in the experimental group was significantly lower than that in the control group,with statistical significance(P<0.05).Conclusion:Anesthesia management under the concept of rapid rehabilitation is applied to patients undergoing laparoscopic cholecystectomy,which can effectively relieve the stress response of patients during perioperative period,shorten the postoperative anesthesia recovery time,exhaust time and the incidence of complications,and help patients quick recovery.
