摘    要:目的观察小青龙汤加减联合有氧运动对慢性阻塞性肺疾病急性加重期(AECOPD)患者肺功能及动脉血气分析指标的影响。方法将60例AECOPD患者按照随机数字表法分为2组。对照组30例予西医常规治疗,治疗组30例在对照组治疗基础上应用小青龙汤加减联合有氧运动治疗。2组均治疗12周。比较2组治疗前后肺功能指标第1 s用力呼气容积(FEV 1)、FEV 1占预计值百分比(FEV 1%)及FEV 1/用力肺活量(FVC)变化;比较2组治疗前后动脉血气分析指标动脉氧分压p(O 2)]、二氧化碳分压p(CO2)]及血氧饱和度变化;比较2组治疗前后6 min步行距离及Borg量表评分变化。结果2组治疗后FEV 1、FEV 1%、FEV 1/FVC均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。2组治疗后p(O 2)、血氧饱和度均较本组治疗前升高(P<0.05),p(CO2)均较本组治疗前降低(P<0.05),且治疗后治疗组p(O 2)、血氧饱和度均高于对照组(P<0.05),p(CO2)低于对照组(P<0.05)。2组治疗后6 min步行距离均较本组治疗前延长(P<0.05),Borg量表评分均较本组治疗前降低(P<0.05),且治疗后治疗组6 min步行距离长于对照组(P<0.05),Borg量表评分低于对照组(P<0.05)。结论小青龙汤加减联合有氧运动治疗AECOPD,能改善患者肺功能及动脉血气,提高运动耐力及生活质量。

Abstract:Objective To observe the effect of modified Xiaoqinglong decoction combined with aerobic exercise on pulmonary function and arterial blood gas in patients with chronic obstructive pulmonary disease(COPD).Methods 60 patients with COPD were divided into two groups according to random number table.30 cases in the control group were treated by western medicine.30 cases in treatment group were treated by modified Xiaoqinglong decoction combined with aerobic exercise on the basis of the treatment in the control group.Both groups were treated for 12 weeks.The changes of forced expiratory volume in the first second(FEV 1),FEV 1 of predicted value(FEV 1%)and FEV 1/forced vital capacity(FVC)were compared before and after treatment in the two groups.The changes of arterial oxygen partial pressurep(O 2)],carbon dioxide partial pressurep(CO 2)]and blood oxygen saturation were compared before and after treatment in the two groups.The difference of 6-minute walking distance and Borg scale score between the two groups before and after treatment were compared.Results The FEV 1,FEV 1%and FEV 1/FVC in the two groups were higher than those before treatment(P<0.05),and the increase was more significant in the treatment group(P<0.05).After treatment,p(O 2)and blood oxygen saturation in the two groups was increased(P<0.05),p(CO 2)was decreased(P<0.05),p(O 2)and blood oxygen saturation in the treatment group were higher than those in the control group(P<0.05),p(CO 2)was lower than those in the control group(P<0.05).The walking distance of 6 minutes after treatment in the two groups was higher than that in the group before treatment(P<0.05),and Borg scale score was lower than that in the group before treatment(P<0.05).The walking distance of 6 minutes after treatment in the treatment group was higher than that in the control group(P<0.05),and Borg scale score was lower than that in the control group(P<0.05).Conclusion The treatment of COPD with the modified Xiaoqinglong decoction combined with aerobic exercise can improve the patient's lung function and arterial blood gas,and can improve the patient's exercise tolerance and quality of life.
