宫颈癌术前新辅助化疗20例疗效观察

目的:研究巨块型宫颈癌术前新辅助化疗(neoadjuvantchemotherapy,NACT)的疗效。方法:2000年1月～2004年2月收治的巨块型宫颈癌20例,采用化疗方案为:顺铂(DDP)20mg/m²,静点,第1～5天;博莱霉素(BLM)10mg/m2,肌注或静点,第1、3、5天;羟基喜树碱(HCPT)6～10mg,静点,第1～5天。化疗后19例行广泛全子宫加盆腔淋巴结清扫术。结果:本组20例巨块型宫颈癌应用NACT治疗,显效率(CR+PR)70%,有效率(CR+PR+MR)95%。其中2例局部肿物基本消失,宫颈仅呈糜烂状,效果甚为理想。结论:术前采用DDP+BLM+HCPT方案化疗,可使宫颈局部肿块缩小或消失,从而提高手术成功率,延长患者的生存期。

An Observation on Curative Effect of 20 Cases of Pre-operation Neoadjuvant Chemotherapy for Bulky Cervical Carcinoma

Objective: To study the observation on the curative effect of pre-operation neoadjuvant chemotherapy for bulky cervical carcinoma. Methods: A total of 20 cases with bulky cervical carcinoma were treated during January 2000 to February 2004. The chemotherapy adopted was DDP 20mg/m2 intravenous injection from 1st to 5th day; BLM 10mg/m2 muscle or intravenous injection at 1st, 3rd and 5th day; HCPT 6-10mg intravenous injection from 1st to 5th day. After chemotherapy, 19 cases were treated with radical surgery. Results: The 20 cases of bulky cervical carcinoma were treated with NACT, the CR+PR was 70%, CR+PR+Impr was 95%. Two cases of them had an ideal effect, the partial tumor was basically eliminated, and only with cervical corruption. Conclusion: Before operation, the chemotherapy plan DDP+BLM+HCPT is adopted, which can reduce or eliminate partial cervical tumors, thus can increase success rate of operation, and can extend the survival period of patients.
